低剂量螺旋CT联合血清VEGF、CEA、CA125水平检测在早期肺癌诊断中的价值

摘　　要：目的 探讨低剂量螺旋CT联合血清血管内皮生长因子(VEGF)、癌胚抗原(CEA)、糖类抗原125(CA125)水平检测在早期肺癌中的诊断价值。方法 选取168例早期肺癌患者为肺癌组,随机选取同期168例肺部良性病变为肺部良性病变组。肺癌组和肺部良性病变组均进行低剂量螺旋CT扫描和血清VEGF、CEA、CA125水平检测,比较肺癌组和肺部良性病变组血清VEGF、CEA、CA125水平、低剂量螺旋CT扫描结果。血清VEGF、CEA、CA125及联合低剂量螺旋CT对早期肺癌诊断效能的ROC曲线特征。结果 早期肺癌患者经低剂量螺旋CT扫描共检出阳性123例。经ROC分析,低剂量螺旋CT诊断早期肺癌的AUC为0.711,敏感度为73.21%,特异度为69.05%。肺癌组的血清VEGF、CEA、CA125水平均高于肺部良性病变组(P<0.05);ROC分析可见,血清VEGF诊断早期肺癌的AUC为0.668,敏感度、特异度分别为48.80%、75.00%;血清CEA诊断早期肺癌的AUC为0.650,敏感度、特异度分别为49.40%、75.60%;血清CA125诊断早期肺癌的AUC为0.730,敏感度、特异度分别为63.70%、76.79%。以血清VEGF、CEA、CA125的截断值为临床值,采用并联的方式进行联合检测,低剂量螺旋CT联合血清诊断早期肺癌的AUC为0.821,敏感度、特异度分别为91.07%、73.21%。结论 低剂量螺旋CT联合血清VEGF、CEA、CA125水平检测早期肺癌的敏感度较高,对诊断早期肺癌有良好应用价值。Objective To explore the diagnostic value of low-dose spiral CT combined with serum vascular endothelial growth factor(VEGF),carcinoembryonic antigen(CEA) and carbohydrate antigen 125(CA125) for early lung cancer.Methods A total of 168 patients with early lung cancer and 168 patients with benign lung lesions were enrolled as lung cancer group and benign lesion group, respectively.All underwent low-dose spiral CT examination and detection of serum VEGF,CEA and CA125.The levels of serum VEGF,CEA and CA125,and examination results of low-dose spiral CT were compared between the 2 groups.The diagnostic efficiency of serum VEGF,CEA,CA125 and low-dose spiral CT for early lung cancer was evaluated by ROC curves.Results In the 168 patients, low-dose spiral CT showed that there were 123 cases with early lung cancer.ROC curves analysis showed that AUC,sensitivity and specificity of low-dose spiral CT in the diagnosis of early lung cancer were 0.711,73.21% and 69.05%,respectively.The levels of serum VEGF,CEA and CA125 in lung cancer group were higher than those in benign lesion group(P<0.05).ROC curves analysis showed that AUC,sensitivity and specificity of VEGF,CEA and CA125 in the diagnosis of early lung cancer were(0.668,48.80%,75.00%),(0.650,49.40%,75.60%) and(0.730,63.70%,76.79%),respectively.AUC,sensitivity and specificity of VEGF combined with CEA,CA125 and low-dose spiral CT in the diagnosis of early lung cancer were 0.821,91.07% and 73.21%,respectively.Conclusion The sensitivity of low-dose spiral CT combined with serum VEGF,CEA and CA125 is high in the diagnosis of early lung cancer, which has good diagnostic value for early lung cancer.
