腹腔镜手术与经阴道手术治疗子宫肌瘤的有效性及安全性比较

摘　　要：目的比较腹腔镜手术与经阴道手术治疗子宫肌瘤的有效性及安全性。方法选取我院2017年1月至2019年12月收治的92例子宫肌瘤患者,采用抽签法将其分为对照组(46例,经阴道手术)和研究组(46例,腹腔镜手术)。比较两组的手术指标、住院天数、血清指标、卵巢功能指标及并发症发生情况。结果研究组的手术时长、住院天数短于对照组,术中出血量少于对照组(P<0.05)。术后3 d,研究组的肿瘤特异性生长因子(TSGF)、胰岛素样生长因子-Ⅰ(IGF-Ⅰ)、前列腺素E_(2)(PGE_(2))水平低于对照组,谷胱甘肽过氧化物酶(GSH-Px)水平高于对照组(P<0.05)。术后1个月,研究组的黄体生成素(LH)、卵泡刺激素(FSH)水平低于对照组,雌二醇(E_(2))水平高于对照组(P<0.05)。研究组的并发症总发生率低于对照组(P<0.05)。结论相较于经阴道手术,腹腔镜手术治疗子宫肌瘤的临床效果更为显著,且安全性更高。Objective To compare the efficacy and safety between laparoscopic surgery and transvaginal surgery in the treatment of hysteromyoma. Methods A total of 92 patients with hysteromyoma admitted in our hospital from January 2017to December 2019 were selected and divided into control group(46 cases, transvaginal surgery) and study group(46cases, laparoscopic surgery) by drawing lots. The surgical indexes, hospitalization days, serum indexes, ovarian function indexes and occurrence of complications were compared between the two groups. Results The operation time and hospitalization days in the study group were shorter than those in the control group, and the intraoperative blood loss was less than that in the control group(P<0.05). Three days after operation, the levels of tumor specific growth factor(TSGF),insulin-like growth factors-Ⅰ(IGF-Ⅰ) and prostaglandin E_(2)(PGE_(2)) in the study group were lower than those in the control group, and the level of glutathione peroxidase(GSH-Px) was higher than that in the control group(P<0.05). One month after operation, the levels of luteinizing hormone(LH) and follicle stimulating hormone(FSH) in the study group were lower than those in the control group, and the level of estradiol(E_(2)) was higher than that in the control group(P<0.05). The total incidence of complications in the study group was lower than that in the control group(P<0.05). Conclusion Compared with transvaginal surgery, laparoscopic surgery has better clinical effect and safety in the treatment of hysteromyoma.
