摘　　要：目的:探讨P21、角蛋白(K R T)7及B C A7联合检测对预测膀胱癌患者预后的效果。方法:回顾性收集2016年2月—2021年2月山东第一医科大学附属人民医院收治的100例膀胱癌患者的临床资料,以免疫组化方法检测P21、KRT7及BCA7在膀胱癌组织以及癌旁组织的表达水平。比较膀胱癌组织及相应的癌旁正常组织P21、KRT7及BCA7表达水平;不同病理特征膀胱癌组织P21、K R T7及B C A7表达水平以及不同预后膀胱癌组织P21、K R T7及B C A7表达水平。结果:膀胱癌组织P21以及KRT7阳性表达率均低于癌旁组织,BCA7阳性表达率高于癌旁组织;低病理分级膀胱癌组织BCA7阴性表达率高于高病理分级,低病理分级膀胱癌组织KRT7阴性表达率低于高病理分级;存活组膀胱癌组织中P21、KRT7的阳性表达率高于死亡组,BCA7的阳性表达率低于死亡组,差异均有统计学意义(P<0.05)。结论:P21、KRT7在膀胱癌组织中阳性表达率较低,BCA7在癌组织中的阳性表达率较高,且BCA7与KRT7在膀胱癌组织中的表达与病理分级有关,且高表达的P21、K R T7以及低表达的B C A7可能意味着较高的存活率,可能是较为理想的膀胱癌预后标志物。Objective To investigate the prognostic effect of P21,keratin(KRT)7 and BCA7 in patients with bladder cancer.Methods Retrospective collection of clinical data of 100 patients with bladder cancer admitted to the People’s Hospital affiliated to Shandong First Medical University from February 2016 to February 2021.Immunohistochemical methods were used to detect the expression levels of P21,KRT7 and BCA7 in bladder cancer tissues and adjacent tissues.The expression levels of P21,KRT7 and BCA7 in bladder cancer tissues and corresponding adjacent normal tissues,the expression levels of P21,KRT7 and BCA7 in bladder cancer tissues with different pathological characteristics and P21,KRT7 and BCA7 in bladder cancer tissues with different prognosis were compared.Results The positive expression rate of P21 and KRT7 in bladder cancer tissues was lower than that in paracancer tissues,and the positive expression rate of BCA7 was higher than that in paracancer tissues.The negative expression rate of BCA7 in bladder cancer with low pathological grade was higher than that with high pathological grade,and the negative expression rate of KRT7 in bladder cancer with low pathological grade was higher than that with high pathological grade.The positive expression rates of P21,KRT7 in bladder cancer tissues of the survival group were higher than those of the death group,The positive expression rates of BCA7 in bladder cancer tissues of the survival group were lower than those of the death group and the difference was statistically significant(P<0.05).Conclusion The positive expression rate of P21 and KRT7 in bladder cancer tissues is low,while the positive expression rate of BCA7 in cancer tissues is high,and the expression of BCA7 and KRT7 in bladder cancer tissues is related to pathological grade,and the high expression of P21 and KRT7 and low expression of BCA7 may mean a higher survival rate.It may be an ideal prognostic marker for bladder cancer.
