摘 要:目的 探讨SS18-SSX和SSX C-末端抗体的表达在滑膜肉瘤(SS)中的诊断价值及预后中的作用。方法 收集徐州医科大学附属医院2010-01—2021-04诊断为SS的病例35例以及其他类型肿瘤40例,免疫组化检测两种抗体在两组肿瘤中的表达,分析它们诊断SS的敏感性和特异性、表达的一致性以及与SS预后的关系。结果 SS18-SSX抗体在80%的SS中呈阳性,对照组均阴性;SSX C-末端抗体在100%的SS中呈阳性,对照组仅两例有不同程度阳性。两种抗体敏感性分别为80%和100%,特异性分别为100%和95%;两者表达一致性较好(Kappa值=0.6,P<0.001),但与SS预后无相关性。结论 SS18-SSX和SSX C-末端抗体对诊断SS具有高度敏感性和特异性,联合检测可替代荧光原位杂交(FISH)用于大多数SS诊断,但对预后判断无显著意义。Objective To investigate the expression, diagnostic value and prognostic role of SS18-SSX antibody and SSX C-terminal antibody in synovial sarcoma(SS). Methods Immunohistochemical(IHC) EnVision method was used to detect the expression of SS18-SSX antibody and SSX C-terminal antibody in 35 cases of SS and 40 cases of other tumors diagnosed at the Department of Pathology, Affiliated Hospital of Xuzhou Medical University from January 2010 to April 2021. The sensitivity, specificity, and consistency of the expression of both the antibodies for diagnosis of SS, and the relationship between both antibody expression and the prognosis of SS were also analyzed. Results SS18-SSX antibody was positive in 80% of SS, but negative in the control group. SSX C-terminal antibody was positive in 100% of SS while only two cases in the control group were positive to varying degrees. The sensitivity of two antibodies was 80% and 100%, and the specificity was 100% and 95%, respectively. Statistics showed that the expression of the two antibodies had high consistency(Kappa =0.6, P<0.001). There was no correlation between their expression and prognosis of SS. Conclusion SS18-SSX antibody and SSX C-terminal antibody have high sensitivity and specificity for the diagnosis of SS. Combined detection can replace fluorescence in situ hybridization in the diagnosis of most SS;however, there is no significant significance in the prognostic evaluation of SS.
