MDM2/CDK4在脂肪肉瘤中蛋白表达及分子遗传特征分析

摘　　要：目的 探讨MDM2/CDK4在脂肪肉瘤(LPS)中蛋白表达及分子遗传特征。方法 收集LPS病例25例,分别采用免疫组化法(IHC)和荧光原位杂交法(FISH)检测MDM2/CDK4在LPS的蛋白表达和基因扩增位情况,分析LPS的病理及分子遗传特征。结果 25例LPS患者年龄22~78岁,中位年龄61岁,男女比例为15∶10,肿瘤直径范围0.8~23 cm,平均直径8.9 cm。肿瘤类型包含ALT/WDLPS、DDLPS、MLPS、PLPS。IHC分别检测MDM2和CDK4蛋白,其阳性率分别为64.0%(16/25)、68.0%(17/25),阳性共表达率为52.0%(13/25)。FISH分别检测MDM2和CDK4基因扩增,其阳性率分别为24.0%(6/25)、24.0%(6/25),共扩增阳性率为24.0%(6/25)。MDM2蛋白的表达与MDM2基因扩增之间不存在相关性(R=0.226,P=0.277),CDK4蛋白的表达与CDK4基因扩增之间不存在相关性(R=-0.016,P=0.939)。结论 在LPS的诊断过程中,MDM2/CDK4蛋白表达可见于多种类型LPS,MDM2/CDK4蛋白表达阳性测情况下须进行FISH检测,FISH检测是LPS辅助诊断的可靠依据。Objective To investigate the protein expression and molecular genetic characteristics of MDM2/CDK4 in liposarcoma(LPS). Methods A total of 25 cases of LPS were collected, and the protein expression and gene amplifications of MDM2/CDK4 in LPS were detected by immunohistochemical method and FISH method, respectively. The pathological and molecular genetic characteristics of LPS were analyzed. Results The age of 25 LPS patients ranged from 22 to 78 years, the median age was 61 years;the male to female ratio was 15∶10;the tumor diameter ranged from 0.8 to 23 cm, with average 8.9 cm. Tumor types included ALT/WDLPS, DDLPS, MLPS, and PLPS. The positive rate of MDM2 and CDK4 protein detected by IHC was 64.0%(16/25) and 68.0%(17/25), respectively, and the total positive expression rate was 52.0%(13/25). The positive rate of MDM2 and CDK4 gene amplification detected by FISH was 24.0%(6/25), 24.0%(6/25), and the positive rate of co-amplification was 24.0%(6/25). There was no correlation between the expression of MDM2 protein and the amplification of MDM2 gene(R=0.226, P=0.277), nor between the expression of CDK4 protein and the amplification of CDK4 gene(R=-0.016, P=0.939). Conclusion In LPS diagnosis, MDM2/CDK4 protein expression can be seen in many types of LPS, and FISH test should be performed when MDM2/CDK4 protein expression is positive, and FISH test is a reliable technique for assisted diagnosis of LPS.
