摘　　要：目的 观察偏头痛患者右向左分流(RLS)的阳性率及分流量大小,分析偏头痛与RLS的关系。方法 纳入129例确诊为偏头痛的患者为病例组,80例健康志愿者为对照组,病例组患者需完成偏头痛伤残评估问卷,应用经颅多普勒(TCD)发泡实验诊断RLS,并对阳性率、分流量进行统计分析。结果 先兆偏头痛组、无先兆偏头痛、对照组RLS阳性率分别为:59.3%,40.0%,23.8%,偏头痛组RLS阳性率显著高于正常对照组。RLS分流量比较中,先兆偏头痛组、无先兆偏头痛、对照组大分流率分别为30.5%,12.9%,3.8%,病例组的大分流率显著高于对照组,差异有统计学意义(P<0.05),3组间中小量分流率无显著差异。Spearman相关分析示偏头痛致残程度与RLS分流量之间无显著相关性(P=0.072)。结论 偏头痛与RLS具有相关性,特别是大量的右向左分流;偏头痛伤残程度与分流量无显著相关。Objective To observe the positive rate and partial flow of right to left shunt(RLS) in patients with migraine, and analyze the relationship between migraine and RLS.Methods One hundred and twenty-nine cases of patient with migraine were included as the case group and eighty cases of healthy volunteer were enrolled as the control group.In the case group, patients diagnosed with RLS by transcranial doppler(TCD) foaming test need to complete the migraine disability assessment questionnaire, and the positive rate and partial flow were statistically analyzed.ResuIts The positive rates of RLS in migraine with aura, migraine without aura and control group were 59.3%,40.0% and 23.8% respectively.The positive rate of RLS in migraine group was significantly higher than that in normal control group.In the comparison of RLS partial flow, the large shunt rates of migraine with aura, migraine without aura and control group were 30.5%,12.9% and 3.8% respectively.The large shunt rate of case group was significantly higher than that of control group(P<0.05).There was no significant difference in medium and small shunt rate among the three groups.Spearman correlation analysis showed that there was no significant correlation between the degree of disability of migraine and RLS partial flow(P=0.072).ConcIusions RLS is associated with a large number of migraines, especially to the left;There was no significant correlation between migraine disability and partial flow.
