摘 要:目的 研究乙肝相关性肝细胞癌(HCC)中乙肝病毒X蛋白(HBx)、微小RNA(miR)-122表达的相关性及临床意义。方法 选择2018年3月至2020年4月期间在焦作煤业集团有限责任公司中央医院接受手术切除的乙肝相关HCC患者作为研究对象,收集HCC组织和癌旁组织,检测HBx的阳性表达率,miR-122、细胞周期蛋白G1(cyclinG1)、X连锁凋亡抑制蛋白(XIAP)、β-连环蛋白(β-catenin)的mRNA表达水平,随访患者的无病生存(DFS)和总生存(OS)情况。结果 HCC组织中HBx的阳性表达率及cyclinG1、XIAP、β-catenin的mRNA表达水平均高于癌旁组织,miR-122的表达水平低于癌旁组织,差异有统计学意义(P<0.05);HBx阳性表达的HCC组织中miR-122的表达水平低于HBx阴性表达的HCC组织,cyclinG1、XIAP、β-catenin的mRNA表达水平高于HBx阴性表达的HCC组织,差异有统计学意义(P<0.05);HCC组织中miR-122的表达水平与cyclinG1(r=-0.361)、XIAP(r=-0.412)、β-catenin(r=-0.391)的mRNA表达水平呈负相关(P<0.05);临床分期Ⅲ~Ⅳ期、合并脉管浸润的HCC组织中HBx的阳性表达率高于临床分期Ⅰ~Ⅱ期、未合并脉管浸润的HCC组织,miR-122的表达水平低于临床分期Ⅰ期、未合并脉管浸润的HCC组织,差异有统计学意义(P<0.05);HBx阳性表达、miR-122表达<中位数的HCC患者累积DFS率、累积OS率均低于HBx阴性表达、miR-122表达≥中位数的HCC患者(P<0.05)。结论 乙肝相关HCC中HBx阳性表达与miR-122表达降低具有相关性,两者共同促进HCC的病理进展及预后不良。Objective To study the correlation of HBx and hepatitis B virus X protein(HBx)and microRNA(miR)-122 expression in hepatitis B related hepatocellular carcinoma(HCC) and its clinical significance. Methods Patients with hepatitis B related HCC who underwent surgical resection in Central Hospital of Jiaozuo Coal Industry Group Co.,Ltd from March 2018 to April 2020 were selected as the research objects. HCC tissues and adjacent tissues were collected for the detection the positive expression rate of HBx,mRNA expression level miR-122,cyclin G1,X-linked inhibitor of apoptosis protein(XIAP),β-catenin. The disease-free survival(DFS)and overall survival(OS)of patients were followed up. Results The positive expression rate of HBx and the mRNA expression levels of CyclinG1,XIAP,β-catenin in HCC tissues were higher than those in adjacent tissues,and the expression level of miR-122 was lower than that in adjacent tissues,the difference was statistically significant(P<0.05). The expression level of miR-122 in HCC tissues with HBx positive expression was lower than that in HCC tissues with HBx negative expression,and the mRNA expression levels of CyclinG1,XIAP,β-catenin were higher than those in HCC tissues with HBx negative expression,the difference was statistically significant(P<0.05). The expression level of miR-122 in HCC tissues was negatively correlated with the m RNA expression level of CyclinG1(r=-0.361),XIAP(r=-0.412)β-catenin(r=-0.391)(P<0.05). The positive expression rate of HBx in clinical stage Ⅲ~Ⅳ HCC tissues with vascular invasion was higher than that in clinical stageⅠ~ⅡHCC tissues without vascular invasion,and the expression level of miR-122 was lower than that in clinical stage Ⅰ ~ Ⅱ without vascular infiltration,the difference was statistically significant(P<0.05). The cumulative DFS rate and cumulative OS rate of HCC patients with HBx positive expression and miR-122 expression < median were lower than those of HCC patients with HBx negative expression and miR-122 expression ≥ median(P<0.05)
