摘 要:目的 探讨胃黄斑瘤与胃泌素17(G-17)、胃蛋白酶原Ⅰ(PGⅠ)、胃蛋白酶原Ⅱ(PGⅡ)及幽门螺杆菌(Hp)的关系。方法 选取2017年2月至2022年3月于合肥市第二人民医院消化内科确诊为胃黄斑瘤的患者75例作为观察组,并选取同期进行胃镜检查并获得正常结果的健康体检者81名作为对照组。比较两组患者的G17、PGⅠ、PGⅡ水平及Hp的感染率。采用多因素Logistic回归分析胃黄斑瘤的独立危险因素,并采用受试者工作(ROC)曲线判断G-17、PGⅠ、PGⅡ、Hp感染对胃黄斑瘤的诊断价值。结果 观察组的G-17、PGⅡ水平显著高于对照组,且PGⅠ、PGⅠ/PGⅡ水平显著低于对照组,差异有统计学意义(t=14.331、8.517、17.986、12.217,P均<0.05)。观察组的Hp阳性率显著高于对照组,差异具有统计学意义(χ^(2)=26.393,P<0.05)多因素Logistic回归分析结果显示,G-17、PGⅠ、PGⅡ水平、Hp感染均为胃黄斑瘤的独立影响因素(P<0.05)。ROC曲线分析结果显示,G-17、PGⅠ、PGⅠ/PGⅡ、Hp单独检测诊断胃黄斑瘤对应的AUC为0.942、0.980、0.938、0.709。结论 G-17、PGⅠ、PGⅡ以及Hp与胃黄斑瘤的发生关系密切,诊断胃黄斑瘤均具有较高的价值,能够作为临床诊断胃黄斑瘤的参考依据。Objective To investigate the relationship between gastric macular tumor and gastrin 17(G-17),pepsinogen I(PG I),pepsinogen Ⅱ(PG Ⅱ)and Helicobacter pylori(Hp). Methods From February 2017 to March 2022,75 patients with gastric macular tumor who were diagnosed in the Department of Gastroenterology of Hefei Second People ’s Hospital were selected as the observation group,and 81 healthy people who had gastroscopy at the same time and obtained normal results were selected as the control group. The levels of G17,PGⅠ,PG Ⅱ and the infection rate of Hp were compared between the two groups. Multivariate logistic regression was used to analyze the independent risk factors of gastric macular tumor,and the diagnostic value of G-17,PG Ⅰ,PG Ⅱ,Hp infection in gastric macular tumor was judged by the receiver operating curve(ROC). Results The levels of G-17 and PG Ⅱ in the observation group were significantly higher than those in the control group,and the levels of PGⅠand PGⅠ/PGⅡwere significantly lower than those in the control group,with a statistically significant difference(t=14.331,8.517,17.986,12.217,P<0.05). The positive rate of Hp in the observation group was significantly higher than that in the control group,and the difference was statistically significant(χ^(2)=26.393,P<0.05). The results of multifactor logistic regression analysis showed that the levels of G-17,PGⅠ,PG Ⅱ and Hp were independent influencing factors of gastric macular tunor(P<0.05). The results of ROC curve analysis showed that the AUCs of G-17,PGⅠ,PGⅠ/PGⅡ,and Hp alone in the diagnosis of gastric xanthoma were 0.942,0.980,0.938,and 0.709,respectively. Conclusion G-17,PGⅠ,PG Ⅱ,and Hp are closely related to the occurrence of gastric macular tumor. They are of high value in the diagnosis of gastric macular tumor and can be used as a reference for clinical diagnosis of gastric macular tumor.
