鼻咽癌合并EB病毒感染患者LMP-1和p53及Ki67与肿瘤恶性进展的关系

摘　　要：目的分析鼻咽癌合并Epstern-Barr(EB)病毒感染患者外周血潜伏膜蛋白1(LMP-1)、p53、Ki67表达与肿瘤恶性进展的关系。方法选取2018年5月-2021年5月河南省人民医院收治的60例鼻咽癌合并EB病毒感染患者纳入感染组,42例鼻咽癌未合并EB病毒感染患者纳入未感染组。比较两组入院时外周血LMP-1、p53、Ki67表达情况,采用Logistic回归方程分析外周血LMP-1、p53、Ki67表达与鼻咽癌合并EB病毒感染的关系,分析外周血LMP-1、p53、Ki67表达与鼻咽癌未合并EB病毒感染患者临床病理特征的关系,根据有无淋巴结转移、远处转移情况将感染组分成不同亚组,比较不同亚组外周血LMP-1、p53、Ki67表达。结果感染组外周血LMP-1、p53、Ki67阳性率高于非感染组(P<0.05);外周血LMP-1、p53、Ki67阳性表达是鼻咽癌患者合并EB病毒感染的风险因素(P<0.05);不同N分期鼻咽癌合并EB病毒感染患者外周血LMP-1、p53、Ki67差异有统计学意义(P<0.05),N分期越高外周血LMP-1、p53、Ki67阳性表达率越高(P<0.05);远处转移鼻咽癌合并EB病毒感染患者外周血LMP-1、p53、Ki67阳性表达率高于未远处转移患者(P<0.05)。结论鼻咽癌合并EB病毒感染患者外周血LMP-1、p53、Ki67阳性率较高,是鼻咽癌患者合并EB病毒的风险因素,且与鼻咽癌合并EB病毒感染患者N分期、远处转移情况密切相关,可能参与肿瘤恶性进展过程。OBJECTIVE To analyze the relationship between expressions of latent membrane protein-1(LMP-1),p53 and Ki67 in peripheral blood and malignant progression of tumor in patients with nasopharyngeal carcinoma complicated with Epstern-Barr(EB)virus infection.METHODS Totally 60 patients with nasopharyngeal carcinoma complicated with EB virus infection who were treated in Henan Provincial People′s Hospital between May 2018 and May 2021 were selected and included in the infection group,and 42 patients with nasopharyngeal carcinoma without EB virus infection were included in non-infection group.The expressions of LMP-1,p53 and Ki67 in peripheral blood at the time of admission were compared between the two groups,and the relationship between expressions of LMP-1,p53 and Ki67 in peripheral blood and nasopharyngeal carcinoma with EB virus infection was analyzed by Logistic regression equation.The relationship between expressions of peripheral blood LMP-1,p53 and Ki67 and clinicopathological characteristics of patients with nasopharyngeal carcinoma without EB virus infection was analyzed.According to the presence or absence of lymph node metastasis and distant metastasis,the infection group was divided into different subgroups,and the expressions of peripheral blood LMP-1,p53 and Ki67 were compared among different subgroups.RESULTS The positive rates of LMP-1,p53 and Ki67 in peripheral blood were higher in infection group than those in non-infection group(P<0.05).The positive expressions of peripheral blood LMP-1,p53 and Ki67 were the risk factors for EB virus infection in patients with nasopharyngeal carcinoma(P<0.05).There were statistical differences in the expressions of peripheral blood LMP-1,p53 and Ki67 among patients with nasopharyngeal carcinoma complicated with EB virus infection of different N stages(P<0.05),and the higher the N stage,the higher the positive expression rates of peripheral blood LMP-1,p53 and Ki67(P<0.05).The positive rates of LMP-1,p53 and Ki67 expression in peripheral blood of patients with dista
