摘　　要：目的回顾性分析神经节细胞胶质瘤继发癫痫患者行外科手术的疗效及预后因素。方法收集昆明三博脑科医院2014年1月至2020年8月期间行外科治疗并经病理证实为神经节细胞胶质瘤的继发性癫痫患者的病例资料进行回顾性分析,定期随访,并通过对患者的病史、影像学表现及手术切除范围等方面进行统计学分析,预测影响癫痫预后的危险因素。结果35例患者均通过手术治疗,术后病理证实均为神经节细胞胶质瘤,平均随访46.67个月,其中28例为EngleⅠ级,7例为EngleⅡ~Ⅲ级,术后复查肿瘤无1例复发,对35例患者的临床特点进行多因素统计学分析,其中病程对于癫痫预后具有统计学意义。结论神经节细胞胶质瘤易继发癫痫,需早期诊断,及早行外科手术治疗,预后较好,其中病程为该病继发癫痫预后的独立危险因素。Objective To retrospectively analyze the efficacy and prognostic factors of surgery in patients with epilepsy secondary to ganglioglioma.Methods Retrospective analysis was conducted on the clinical data of patients epilepsy secondary to ganglioglioma who underwent surgery and pathologically confirmed to be ganglioglioma in Kunming Sanbo Brain Hospital from January 2014 to August 2020.Regular follow-up was performed.The risk factors affecting the prognosis of epilepsy were predicted by statistical analyzing the patient’s medical history,imaging manifestations,surgical resection scope,etc.Results There were 35 patients who underwent surgery and confirmed as ganglioglioma by postoperative pathology.The mean follow-up was 46.67 months.Among them,28 patients achieved EngleⅠclass outcome,and 7 patients achieved EngleⅡ-Ⅲclass outcome.There was no recurrence of tumor after surgery.The clinical characteristics of 35 patients were analyzed by multivariate statistics.The disease course was statistically significant for the prognosis of epilepsy.Conclusion Ganglioglioma is prone to induce secondary epilepsy.Early diagnosis and early surgery are required for good prognosis.The disease course is an independent risk factor for the prognosis of secondary epilepsy to ganglioglioma.
