健肝乐颗粒联合复方蛋氨酸胆碱片治疗酒精性脂肪肝对氧化应激和细胞因子的影响

摘　　要：目的探讨健肝乐颗粒联合复方蛋氨酸胆碱片治疗酒精性脂肪肝对氧化应激和细胞因子的影响。方法选取2019年5月至2021年5月武汉市精神卫生中心收治的酒精性脂肪肝患者108例作为研究对象,采用随机数字表法分为对照组与观察组,各54例。对照组给予常规西药复方蛋氨酸胆碱片治疗,观察组在其基础上联合健肝乐颗粒治疗,两组均连续治疗3个月,比较两组患者临床疗效、治疗前后肝功能[丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)、谷氨酰转肽酶(GGT)]、氧化应激指标[超氧化物歧化酶(SOD)、丙二醛(MDA)]、细胞因子[肿瘤坏死因子-α(TNF-α)、成纤维细胞生长因子21(FGF-21)、白细胞介素-33(IL-33)]水平,同时观察治疗前后肝脏超声指标变化以及治疗期间不良反应。结果观察组治疗有效率高于对照组,差异有统计学意义(P<0.05)。治疗后,观察组与对照组相比AST、ALT及GGT较低,SOD较高,MDA、TNF-α、FGF-21及IL-33较低,肝脏脾大、管道结构显示不清、远端回声衰减及近端弥漫点状高回声所占比例、肝弹值均较低,差异有统计学意义(P<0.05)。两组治疗期间均未出现明显的不良反应。结论健肝乐颗粒联合复方蛋氨酸胆碱片治疗酒精性脂肪肝临床效果显著,可有效改善患者氧化应激以及细胞因子水平,提高肝功能,且治疗期间未增加不良反应,安全性较高。Objective To explore the effect of Jianganle Granules combined with Compound Methionine Choline Tablets on oxidative stress and cytokines in the treatment of alcoholic fatty liver disease.Methods A total of 108 patients with alcoholic fatty liver disease who were treated in Wuhan Mental Health Center from May 2019 to May 2021 were selected and randomly divided into control group and observation group,54 cases in each group.The control group was given conventional western medicine compound methionine bile.On the basis of the treatment with alkali tablets,the observation group was treated with Jianganle granules in combination.Both groups were treated for 3 consecutive months.The clinical efficacy,liver function[alanine aminotransferase(ALT),daily Aspartate aminotransferase(AST),glutamyl transpeptidase(GGT)],oxidative stress indicators[superoxide dismutase(SOD),malondialdehyde(MDA)],cytokines[tumor necrosis factor-α(TNF-α),fibroblast growth factor 21(FGF-21),interleukin-33(IL-33)]levels,and the changes of liver ultrasound indexes and adverse reactions during treatment were observed in patients of the two groups before and after treatment.Results The effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,AST,ALT,GGT,SOD,MDA,TNF-αin the observation group were lower than those in the control group,FGF-21 and IL-33 were lower,the liver and spleen were larger,the pipeline structure was unclear,the proportion of distal echo attenuation and proximal diffuse dot hyperecho,and the liver elastic value were lower,the difference was statistically significant(P<0.05).There were no obvious adverse reactions in the two groups during treatment.Conclusion The clinical effect of Jianganle granules combined with compound methionine choline tablets in the treatment of alcoholic fatty liver is significant,which can effectively improve the patient's oxidative stress and cytokine levels,improve liver function,and there is no increase in
