摘  要:目的基于2021年上海市二级以上医院营养科质控指标数据,评价医院营养食堂在不同管理模式下治疗膳食工作的开展情况。方法收集2021年97家上海市二级以上不同等级医院营养科质控数据,按医院营养食堂管理模式分为营养科组、外包管理组、后勤管理组,对比3组间不同等级医院的占比情况及质控指标达标率。结果三级医院与二级医院之间营养食堂管理模式的构成比不同,差异有统计学意义(P<0.05);质控指标评价中营养科组膳食系统构建的达标率为39.2%,明显高于外包管理组和后勤管理组的19.6%、3.1%,差异有统计学意义(P<0.05);后勤管理组中膳食质控小组建立的达标率为5.2%,低于营养科组和外包管理组的42.3%、30.9%,差异有统计学意义(P<0.05)。结论医院营养食堂的管理模式与医院等级存在联系;但不同管理模式下营养食堂的治疗膳食管理工作还存在问题与不足,尚需持续性改进。Objective To evaluate the therapeutic diets in nutrition-promoting canteens under different management modes based on the quality control indicator data obtained from the departments of nutrition at secondary or tertiary hospitals in Shanghai in 2021.Methods The quality control indicator data were collected from the departments of nutrition at 97 secondary or tertiary hospitals in Shanghai in 2021.The nutrition-promoting canteens were divided into nutrition department management group,outsourcing management group,and logistics management group based on the management modes.The proportions of these management modes and the compliance rate of quality control indicators in hospitals of different grades were compared among these three groups.Results The constituent ratios of different management modes showed significant difference between tertiary hospitals and secondary hospitals(P<0.05).The compliance rate of“Dietary system construction”was 39.2% in the nutrition department management group,significantly higher than those in outsourcing management group(19.6%)and logistics management group(3.1%)(both P<0.05).The compliance rate of“Establishment of a dietary quality control team”was 5.2%in the logistics management group,significantly lower than those in the nutrition department management group(42.3%)and the outsourcing management group(30.9%)(both P<0.05).Conclusions The management mode of nutrition-promoting canteens relates to hospital levels.However,management of therapeutic diets in nutrition-promoting canteens under different management modes still faces many challenges and needs to be further optimized.
