摘　　要：目的研究PD-L1的表达状态与临床病理特征的相关性,分析PD-L1的表达对EGFR-TKIs治疗晚期非小细胞肺癌预后的预测价值。方法收集2019年1月1日~2020年12月31日在潍坊市人民医院接受EGFR-TKI靶向治疗并在治疗前行PD-L1检测的非小细胞肺癌患者的临床资料进行回顾性分析。结果在46例符合纳入标准的非小细胞肺癌患者中,21例PD-L1阳性,25例PD-L1阴性。PD-L1的表达与年龄、性别、吸烟史、肿瘤家族史、临床分期、体能状况ECOG评分、分化程度无关(P>0.05)。PD-L1阳性表达是EGFR-TKIs治疗组患者的不良预后因素。两组的中位PFS值分别为10.6个月和14.9个月,不同PD-L1分组间的生存函数曲线差异有统计学意义(P=0.047)。结论(1)PD-L1的表达与年龄、性别等临床病理特征无关;(2)PD-L1阳性预示着NSCLC恶性程度更高,预后更差,且对EGFR-TKIs的效果不佳。Objective To investigate the correlation between the expression status of PD-L1 and the clinicopathological features,and to analyze the predictive value of PD-L1 expression in the prognosis of EGFR-TKIs in the treatment of advanced non-small cell lung cancer.Methods From January 1,2019 to December 31,2020,the clinical data of patients with non-small cell lung cancer who received EGFR-TKI targeted therapy in Weifang People’s Hospital and were tested for PD-L1 before treatment were retrospectively analyzed.Results Among the 46 patients with non-small cell lung cancer who met the inclusion criteria,21 patients were positive for PD-L1 and 25 patients were negative for PD-L1.The expression of PD-L1 was not related to age,sex,smoking history,tumor family history,clinical stage,physical fitness status ECOG score,and degree of differentiation(P>0.05).Positive expression of PD-L1 was a poor prognostic factor in patients in the EGFR-TKIs treatment group.The median PFS values of the two groups were 10.6 months and 14.9 months,respectively,and the difference in survival function curves between different PD-L1 groupings was statistically significant(P=0.047).Conclusions(1)The expression of PD-L1 is not related to the clinicopathological features such as age and sex;(2)A positive PD-L1 indicates a higher degree of NSCLC malignancy,a worse prognosis,and a poor effect on EGFR-TKIs.
