摘 要:目的研究不同程度阻生的下颌第三磨牙患者的颅颌面结构特点。方法对98例下颌第三磨牙阻生患者的头颅侧位片进行测量分析,按照全景片双侧下颌第三磨牙的阻生状态进行分组:低位阻生组(组1):下颌第三磨牙最高点低于邻牙牙颈部;中位阻生组(组2):下颌第三磨牙最高点高于邻牙牙颈部但低于牙弓平面;高位阻生组(组3):下颌第三磨牙最高点平行或高于牙弓平面。对3组之间的头影测量数据进行统计分析,以期得到下颌第三磨牙的不同程度阻生与患者颅颌面结构之间的关系。结果低位阻生组患者的前颅底长度(S-N)、全颅底长度(N-Ba)、下面高(ANS-Me)、全面高(N-Me)明显大于中位阻生组和高位阻生组。低位阻生组的颅底角(∠N-S-Ba)明显大于高位阻生组,而下颌骨突度(∠S-N-Pog,∠S-N-B)明显小于高位阻生组。结论下颌第三磨牙阻生程度与颅颌面结构的生长发育有关,尤其是颅底长度和面部高度。Objective To study the craniofacial structures in patients with different degree of third molar impaction.Methods Lateral cephalographs of 98 patients with impacted third molar were analyzed.Patients were divided into three groups.Group 1:third molar occlusal height is below the cervical margin of adjacent second molar(Class C);Group 2:third molar occlusal height is between the occlusal level and cervical level of adjacent second molar(Class B);Group 3:third molar occlusal height is level with the adjacent second molar(Class A).Adjusted means for craniofacial measurements among groups were compared using generalized linear models(SPSS,version 16.0;IBM,Armonk,NY)after being adjusted for age and gender effects.Results The anterior and total cranial base length(S-N,N-Ba),lower and full facial height(ANS-Me,N-Me)were markedly longer in group 1 compared to group 2 and 3.∠N-S-Ba was significantly larger in group 1 than in group 3.∠S-N-Pog,∠S-N-B and the angle of NP to FH were significantly smaller in group 1 than in group 3.Conclusion The mandibular third molar impaction is associated with growth and development of craniofacial morphology,especially in the cranial base and facial height.
