中西医结合联合心理治疗对原发性闭角型青光眼患者术后高眼压的影响

摘　　要：目的观察中西医结合联合心理治疗对原发性闭角型青光眼患者术后高眼压的影响。方法选取原发性闭角型青光眼术后高眼压患者102例,按照随机数字表法分为对照组51例(92眼)和治疗组51例(90眼)。对照组患者术后予常规西医治疗及临床护理指导,治疗组在对照组基础上加用中药、针刺治疗及心理治疗。比较2组治疗前后焦虑自评量表(SAS)、抑郁自评量表(SDS)评分及视力、眼压、视野变化情况。结果治疗组最佳矫正视力(BCVA)改善有效率83.3%(75/90),对照组为43.5%(40/92),治疗组BCVA改善有效率高于对照组(P<0.05)。治疗后,2组SDS、SAS评分均较本组治疗前降低(P<0.05),且治疗组低于对照组(P<0.05)。治疗后,2组眼压和平均光敏感度(MS)、平均缺损值(MD)均较本组治疗前明显改善(P<0.05),且治疗组改善优于对照组(P<0.05)。结论中西医结合协同心理治疗的综合干预可以明显把原发性闭角型青光眼术后高眼压患者的眼压控制在目标范围内,提升患者视力,改善视野范围,稳定患者情绪。Objective To observe the effect of integrative medicine combined with psychotherapy on postoperative intraocular hypertension in patients with primary angle-closure glaucoma(PACG).Methods A total of 102 PACG patients with postoperative intraocular hypertension were randomly assigned into the treatment group(n=51,90 eyes)and the control group(n=51,92 eyes).All patients were managed by conventional Western medicine and clinical nursing,and those in the treatment group were additionally treated with traditional Chinese medicine,acupuncture and psychotherapy.The scores of Self-rating Anxiety Scale(SAS)and Self-rating Depression Scale(SDS),vision,intraocular pressure(IOP),visual field(VF)were included as comparators.Results The improvement rate of best corrected visual acuity(BCVA)in the treatment group was better than that in the control group(83.3%[75/90]vs 43.5%[40/92],[P<0.05],respectively).After treatment,SDS and SAS scores in groups were decreased(P<0.05),which were decreased in the treatment group compared with the control group(P<0.05);IOP,mean sensitivity(MS)and mean defect(MD)in groups were significantly improved(P<0.05),improvement of which in the treatment group were significantly better(P<0.05).Conclusion For PACG patients with postoperative intraocular hypertension,integrative medicine combined with psychotherapy can significantly control IOP within the target range,improve vision and VF,and stabilize emotions.
