从脑玄府论治脑血管周围间隙扩大探讨

摘　　要：基于脑玄府理论,认为脑血管周围间隙扩大的基本病机是玄府郁闭、脑络瘀阻;病位在脑,涉及脾肝肾。病机表现为玄府瘀滞和玄府空虚。玄府是气、血、精、神、津、液的通道,玄府瘀滞,具体表现气滞、痰凝、血瘀。玄府因虚为病,可表现为各脏腑气虚、血虚、津少。脑血管周围间隙扩大的治疗以开通玄府为总原则,脑玄府空虚则治以补虚开玄;脑玄府瘀滞则治以祛邪开玄,根据病邪的不同,予以行气、祛痰、化瘀。无论病性的虚实,均辅以芳香类中药达到引药上行、行气、化湿、开窍之功。Based on the theory of brain’s Xuanfu,obstruction of Xuanfu and stasis of brain collaterals are the basic pathogenesis of widening of Virchow-Robin spaces,and the lesion location is in brain,but which is involved with spleen,liver and kidney.The pathogenesis is manifested as stasis of Xuanfu and voidness-deficiency of Xuanfu.Xuanfu is the channel of qi,blood,essence,mind,body fluid and liquid.The stasis of Xuanfu is specifically manifested as qi stagnation,phlegm coagulation and blood stasis.Xuanfu is sick due to deficiency,which can be manifested as qi deficiency of each Zang-fu organ,blood deficiency and lack of body fluid.The treatment of widening of Virchow-Robin spaces should be based on the general principle of opening Xuanfu,while the voidness-deficiency of brain’s Xuanfu is treated based on reinforcing deficiency to open Xuanfu,and the stasis of brain’s Xuanfu is treated based on eliminating pathogens to open Xuanfu.Promoting qi,dispelling phlegm and removing blood stasis can be performed according to the different pathogenic factors.Whether the natur of disease is deficiency or excess,the aromatic Chinese herbal medicine should be supplemented,in order to achieve the efficacy of inducting the medicines to ascend,promoting qi,removing dampness and opening Xuanfu.
