鸡屎藤治疗风湿痹病135例临床经验体会

摘　　要：目的:总结运用鸡矢藤治疗风湿痹病的临床经验。方法:凡是肢体关节、筋骨、肌肉等部位疼痛,按中医辨病为风湿痹病的患者,在辨证的基础上均用鸡屎藤经验方(鸡屎藤30~60 g)加减治疗,并以适量鲜药或饮片打粉煎煮外敷。结果:135例疗效显著,治愈率为77.04%(104/135),好转率为22.96%(31/135),总有效率为100%(135/135),未见不良反应。结论:鸡屎藤具有良好的临床推广价值,首先,鸡屎藤药效突出,具有良好的祛风除湿、止疼痛、消肿胀、解毒、健胃消食功效,经内服直达病变深在部位,外敷直接作用于浅表病变部位,内服外敷,祛风湿止痛疗效显著。其次,可提高患者的用药依从性,特别是久用风湿类西药伤胃的患者,常伴见胃肠痉挛性疼痛,导致服用风湿类西药依从性差,难于坚持,而以鸡屎藤为主的经验方不伤胃,且健胃消食止胃肠痉挛性疼痛疗效亦佳,从而使患者依从性大大提高,这对治愈风湿痹病具有明显的优势。再次,鸡屎藤药性甘、微苦、平,无论寒热虚实皆可用,药性平和、无特殊不良反应宜于久用,且鸡屎藤药源广泛,易于获取和加工,四季皆可采收,且价格便宜,这对广大的农村患者而言具有极大的现实意义。Objective: To summarize the clinical experience of treating rheumatic arthralgia disease with Jishiteng(Paederia foetida L. J), the whole herb of Paederia foetida(Lour.) Methods: All patients with pain in limbs, joints, muscles, bones and other parts, who are diagnosed as rheumatic arthralgia, are treated with modified JST empirical formula(30-60 g, JST) on the basis of TCM syndrome differentiation, and an appropriate amount of fresh medicine or powder of JST are used for topical application. Results: 135 cases had significant curative effect, the curative ratio was 77.04%(104/135), the improvement rate was 22.96%(31/135), the total efficiency was 100%(135/135), and no adverse reaction was observed. Conclusion: JST has good clinical promotion value. First of all, JST has outstanding efficacy, which has good effects of dispelling wind and dehumidification, relieving pain and swelling, detoxification, and strengthening stomach and digestion. By oral administration, it can reach the deep part of the lesion, and external application can directly act on the superficial part of the lesion. By oral administration and external application, it has significant effect of dispelling wind and dampness and relieving pain. Secondly, it can improve the compliance of patients with medication, especially those who have used Western rheumatic drugs for a long period, their stomachs are hurt with gastrointestinal spastic pain, resulting in poor compliance of taking Western rheumatic drugs, while the empirical formula based on JST does not hurt their stomachs, and the effect of strengthening stomach and eliminating gastrointestinal spastic pain is also good, so that patients’ compliance is greatly improved, which has obvious advantages in curing rheumatic arthralgia. Thirdly, the medicinal properties of JST are sweet, slightly bitter and mild, which can be used to treat cold or heat, deficiency or excess syndromes.
