健脾理气汤灌肠对胃癌术后并发胃瘫综合征患者细胞免疫功能、胃肠功能及肠内营养状况的影响研究

摘　　要：目的:探讨并分析健脾理气汤灌肠对胃癌术后并发胃瘫综合征患者细胞免疫功能、胃肠功能及肠内营养状况的影响。方法:本次研究选取2018年10月-2020年10月接收的胃癌术后并发胃瘫综合征患者160例为研究对象,按照随机数字表法分为两组,各80例,对照组接受常规肠内营养支持治疗,试验组在对照组的基础上行健脾理气汤灌肠治疗,对比两组治疗前后血清营养各项指标、中医证候积分、免疫功能及胃肠功能各项指标情况。结果:治疗前两组患者血清白蛋白(Albumin,ALB)、前白蛋白(Prealbumin,PAB)、转铁蛋白(Transferrin,TRF)水平的差异不具有统计学意义(P>0.05);试验组治疗后血清ALB、PAB、TRF水平均显著高于对照组(P<0.05);治疗前两组患者胃痛胃胀、呕吐嗳气、便结黑便等中医证候积分情况的差异均不具有统计学意义(P>0.05);治疗后试验组胃痛胃胀、呕吐嗳气、便结黑便等中医证候积分情况均优于对照组(P<0.05)。结论:健脾理气汤灌肠肠内营养治疗对胃癌术后并发胃瘫综合征患者的疗效显著,可有效改善患者的营养状况及免疫功能,安全性好,还可减少不良反应的发生,具有较好的临床应用价值,应多加以推广应用。Objective:To explore and analyze the effect of the Jianpi Liqi decoction(健脾理气汤)enema on cellular immune function,gastrointestinal function and enteral nutrition in patients with gastroparesis syndrome after gastric cancer surgery.Methods:A total of 160 patients with gastroparesis syndrome after gastrectomy for gastric cancer from October 2018 to October 2020 were randomly divided into two groups,80 cases in the control group were treated by routine enteral nutrition support,and the other 80 cases in the test group were treated with the Jianpi Liqi decoction enema on the basis of the treatment in the control group.Serum nutrition indexes,TCM syndrome scores,immune function and gastrointestinal function indexes in the two groups were compared before and after treatment.Results:There was no significant difference in the levels of ALB,PAB and TRF between the two groups before treatment(P>0.05),but the levels of ALB,PAB and TRF in the test group were significantly higher than those in the control group after treatment(P<0.05).Before treatment,there was no significant difference in the scores of TCM symptoms such as stomachache,stomach distension,vomiting,belching and black stool between the two groups(P>0.05);after treatment,the scores of stomachache,gastric distension,vomiting,belching and black stool in the test group were better than those in the control group(P<0.05).Conclusion:The Jianpi Liqi decoction enema enteral nutrition therapy is effective on gastric cancer in patients with gastroparesis syndrome after surgery,it can effectively improve the nutritional status and immune function,has good safety,and can reduce the occurrence of adverse reactions in patients.It has good clinical application value,and should be popularized and applied more to improve the therapeutic effect.
