摘　　要：目的:探讨轻中度缺血性卒中后认知障碍(PSCI)患者血清miR-210-3p的表达水平及与认知障碍的相关性。方法:选取2019年1月—2021年11月某院收治的轻中度缺血性卒中后认知障碍患者30例为研究组,并选取同期该院健康体检者30例作为对照组。采用蒙特利尔认知量表(MoCA)对受试者进行认知评估,采用荧光定量PCR检测患者血清中miR-210-3p表达水平,针对miR-210-3p表达水平与MoCA评分进行相关性分析。采用受试者工作特征曲线(ROC)评估miR-210-3p表达水平对PSCI的诊断价值。结果:研究组患者血清中miR-210-3p表达水平为(0.045±0.035),低于健康对照组的(0.380±0.475),差异有统计学意义(P<0.001);研究组患者血清中miR-210-3p表达水平与MoCA评分呈正相关性(R=0.486,P<0.05);ROC曲线分析表明,miR-210-3p在轻中度缺血性PSCI中具有良好的诊断性能,灵敏度0.815,特异度0.875,曲线下面积(AUC)为0.937(95%CI 0.831~0.986),截断值0.095(P<0.01)。结论:miR-210-3p在轻中度缺血性PSCI血清中的表达是下降的,与MoCA评分呈正相关性,可作为PSCI的潜在分子生物标志物,而且可能为该病的预防和治疗提供新的见解。Objective:To investigate the expression level of serum microRNA(miR)-210-3b in patients with mild to moderate ischemic post-stroke cognitive impairment(PSCI)and its correlation with cognitive impairment.Methods:A total of 30 patients with mild to moderate ischemic post-stroke cognitive impairment who were admitted to a hospital from January 2019 to NovembeR2021 were selected foRthe study,and 30 healthy people by physical examination in the hospital during the same period were selected as the control group.Cognitive function was evaluated by Montreal Cognitive Assessment(MoCA).The expression level of miR-210-3b were measured by Real-time PCR.The correlation between miR-210-3p expression levels and MoCA score was analyzed.Results:The expression level of miR-210-3p in serum of the study group was(0.045±0.035),which was significantly loweRthan that in the healthy control group(0.380±0.475),the difference being statistically significant(P<0.001).The expression level of miR-210-3p of the study group was positively correlated with MoCA(R=0.486,P<0.05).The ROC curve analysis showed that miR-210-3p presented excellent diagnostic performance,with sensitivity being 0.815 and specificity 0.875.The area undeRthe curve(AUC)was 0.937(95%CI:0.831~0.986),and the cutoff value was 0.095(P<0.01).Conclusion:The expression level of miR-210-3b in serum of mild to moderate PSCI patients is decreased,and is positively correlated with MoCA score,which may be a promising biomarkeRfoRPSCI,and may provide new insights foRthe prevention and treatment of PSCI.
