手法康复结合电针治疗脑卒中后复杂性区域性疼痛综合征的临床观察

摘　　要：目的:观察手法康复结合电针治疗对脑卒中后复杂性区域性疼痛综合征(CRPS)的影响。方法:采用随机数字表法将2019年6月-2020年6月于南京市秦淮区中医医院康复科就诊的60例脑卒中后CRPS患者分为观察组(n=30)与对照组(n=30)。对照组予以常规基础治疗联合手法康复训练治疗,观察组在对照组治疗方案基础上加用电针疗法。观察两组治疗前后视觉模拟评分法(VAS)、运动功能状态量表(MSS)、Brunnstrom分期、改良Barthel指数(MBI)的差异,以评价患者疼痛程度、运动能力及日常生活能力。结果:治疗后观察组VAS评分低于对照组(P<0.05);治疗后观察组MSS评分高于对照组(P<0.05);治疗后观察组上肢、手的Brunnstrom分期均较对照组高(P<0.05);治疗后观察组MBI评分高于对照组(P<0.05)。结论:手法康复结合电针治疗脑卒中后CRPS可显著减轻患者疼痛,改善手部及上肢运动功能,提高日常生活能力。Objective:To observe the effect of manual rehabilitation combined with electro-acupuncture on post-stroke complex regional pain syndrome(CRPS).Method:A total of 60 patients with post-stroke CRPS who visited the Rehabilitation Department of Nanjing Qinhuai District Hospital of Traditional Chinese Medicine from June 2019 to June 2020 were divided into an observation group(n=30) and a control group(n=30) according to the random number table method.The control group was treated with routine basic treatment combined with manual rehabilitation training,and the observation group was treated with electro-acupuncture on the basis of the treatment plan of the control group.The differences of visual analogue scale(VAS) scores,motor status scale(MSS) scores,Brunnstrom stages and modified barthel index(MBI) between the two groups before and after treatment were observed in order to evaluate the pain degree,exercise ability and daily living ability of patients.Result:After treatment,the VAS score of the observation group was lower than that of the control group(P<0.05).After treatment,the MSS score of the observation group was higher than that of the control group(P<0.05).After treatment,Brunnstrom stages of upper limbs and hands in the observation group were higher than those in the control group(P<0.05).After treatment,the MBI score of the observation group was higher than that of the control group(P<0.05).Conclusion:Manual rehabilitation combined with electro-acupuncture can significantly relieve the pain of patients with post-stroke CRPS,improve the motor function of hands and upper limbs,and improve the daily living ability.
