HR-HPV联合TCT检查在早期宫颈癌前病变中的诊断价值研究

摘　　要：目的探究在宫颈癌前病变中应用高危型人乳头瘤病毒(HR-HPV)联合液基薄层细胞学检查(TCT)的诊断价值。方法40例疑似宫颈癌前病变患者,均进行TCT、HR-HPV单独及联合检查,根据阴道镜病理检查结果,分析不同检查方法的诊断效能(灵敏度、特异度及准确率)。结果40例患者经阴道镜病理检查显示,22例患者为宫颈癌前病变阳性,占比为55.00%;18例患者为宫颈癌前病变阴性,占比为45.00%。HR-HPV单独检测的灵敏度、特异度、准确率分别为90.91%、50.00%、72.50%,TCT单独检测的灵敏度、特异度、准确率分别为63.64%、33.33%、50.00%,HR-HPV+TCT检测的灵敏度、特异度、准确率分别为95.45%、88.89%、92.50%。HR-HPV单独检测与HR-HPV+TCT检测的灵敏度、准确率均高于TCT单独检测,差异具有统计学意义(P<0.05);HR-HPV+TCT检测的特异度均高于HR-HPV、TCT单独检测,准确率高于HR-HPV单独检测,差异具有统计学意义(P<0.05)。结论在对宫颈癌前病变患者进行筛查时,选择TCT联合HR-HPV检查具有较高的诊断准确率,有助于帮助医务人员了解患者的病情。Objective To investigate the diagnostic value of high-risk human papillomavirus(HRHPV)combined with thin-layer cytology testing(TCT)in early-stage cervical precancerous lesions.Methods 40 patients with suspected cervical precancerous lesions were examined by TCT and HR-HPV alone and in combination,and the diagnostic efficacy(sensitivity,specificity and accuracy)of the different screening methods was analyzed according to the results of colposcopic pathology.Results Colposcopic pathological examination of 40 patients showed that 22 patients(55.00%)were positive for cervical precancerous lesions and 18 patients(45.00%)were negative for cervical precancerous lesions.The sensitivity,specificity,and accuracy of HR-HPV alone were 90.91%,50.00%,and 72.50%;the sensitivity,specificity,and accuracy of TCT alone were 63.64%,33.33%,and 50.00%;and the sensitivity,specificity,and accuracy of HR-HPV+TCT were 95.45%,88.89%,and 92.50%.The sensitivity and accuracy of HR-HPV alone and HR-HPV+TCT were higher than those of TCT alone,and the difference was statistically significant(P<0.05).The specificity of HR-HPV+TCT were higher than those of HR-HPV and TCT alone,and the accuracy was higher than HR-HPV alone,and the difference was statistically significant(P<0.05).Conclusion In the screening of cervical precancerous lesions,TCT combined with HRHPV has a high diagnostic accuracy,which can help medical staff to understand the patient’s condition.
