摘　　要：目的 探讨老年女性乳腺癌手术以后改进乳腺内外侧斜位摄影方式和临床应用价值。方法 对94例老年女性乳腺癌术后患者进行常规乳腺复查,然后通过试验进行对照研究,从而将常规摄影位和改进后的内外侧斜位进行对比,找出存在的不同之处。对改进的内外侧斜位与常规内外侧斜位图像采用Wilcoxon秩和检验进行统计学研究,根据老年女性乳腺癌患者乳腺切口位置的不同分组进行,并对改进的内外侧斜位与常规内外侧斜位图像由2位阅片者进行双盲打分比较。结果 老年女性乳腺癌腺体切口在内上方者,切线位乳头评分提高;老年女性乳腺癌腺体切口在外上方者,改进后的内外侧斜位的乳腺腺体显露程度和切线位乳头评分都高于常规内外侧斜位,对比常规内外侧斜位,改进后的内外侧斜位图像评分提高明显,差异有统计学意义,P<0.05。结论对老年女性乳腺癌患者的乳腺进行检查时,可以通过体位内外倾斜来改进内外侧斜位的摄影方式,从而使图像质量显著提高。Objective To explore a improved performance of MLO projection in mammography after breast cancer in older women surgery, to evaluate the clinical application value of the MLO method. Methods Ninety-four patients with history of breast cancer in older women surgery and with routine follow-up for mammography examinations were enrolled in this study. Self-controlled experiment was carried to compare the differences between the modified MLO projection position and the traditional position. Paired Wiloxon Signed Ranks test was used to compaye differences in mammography between modified MLO position and traditional MLO position, patiens were divided into subgroups by position of breast incision. Paired MLO images were blindly evaluated by two radiologic technologists. Results For elderly women with breast cancer with upper-medial incisions, scores of the tangent position of the nippe were improved. For elderly women with breast cancer with upper-lateral incisions, the modified MLO position showed signilicantly improved scores in the tangent position of the nipple and breast tissues exposure compared with that in traditional MLO position. The modified MLO position showed significantly increased three point scores compared with traditional MLO position. Conclusion For elderly women with breast cancer, it is supposed that mammographic images with high quality be improved based on modified MLO position method. It can provide important implications for the clinical diagnosis and help to accurate treatment for elder women with breast cancer.
