冰冻切片对甲状腺微小乳头状癌术中病理诊断的准确性及价值

摘　　要：目的分析冰冻切片在甲状腺微小乳头状癌术中病理诊断中的准确性及应用价值。方法选取2021年1月—2022年10月在东莞东华医院接受治疗的甲状腺微小乳头状癌患者50例,所有研究对象均予以术中冰冻切片和术后石蜡切片病理检查,并以术后石蜡切片结果为诊断金标准,分析术中冰冻切片对该疾病的诊断准确率。结果肉眼可见大多数甲状腺微小乳头状癌组织为灰红色,其中见大小不等的灰白色结节,边界欠清。部分患者可见多个结节状肿物,切面灰白者质硬,紧邻表面被膜;切面半透明者边界均尚清,质中-硬,部分半透明者呈胶冻样。以术后石蜡病理学检查结果为金标准,术中冰冻切片检查准确48例,误诊1例,漏诊1例,诊断准确率为95.00%。结论冰冻切片在甲状腺微小乳头状癌术中病理诊断中的准确性较高,具有临床参考价值。Objective To analyze the accuracy and application value of frozen section in pathologic diagnosis of thyroid micropapillary carcinoma.Methods A total of 50 patients with thyroid micropapillary carcinoma who were treated in Dongguan Tungwah Hospital from January 2021 to October 2022 were selected.All subjects were examined by intraoperative frozen section and postoperative paraffin section pathology.The diagnostic accuracy of intraoperative frozen section was analyzed based on the results of postoperative paraffin section.Results Most of the papillary thyroid microcarcinoma tissues were gray-red,and there were gray-white nodules of varying sizes with blurred boundaries.In some patients,multiple nodular masses can be seen;the section surface is hard and close to the surface capsule;the section surface is translucent and the boundary is clear,medium to hard,and the part translucent is jelly-like.With the results of postoperative paraffin pathology as the gold standard,the intraoperative frozen section examination was accurate in 48 cases,misdiagnosed in 1 case and missed in 1 case,with the diagnostic accuracy of 95.00%.Conclusion The accuracy of frozen section in the pathologic diagnosis of thyroid micropapillary carcinoma is high,and it has the clinical reference value.
