达芬奇机器人手术在妇科恶性肿瘤应用中的临床效果分析

摘　　要：目的探讨妇科恶性肿瘤患者行达芬奇机器人辅助腹腔镜手术治疗的可行性及安全性,为临床实践中手术方式的确定提供参考依据。方法回顾性分析107例妇科恶性肿瘤患者,按照手术方式不同分为机器人手术组(52例)和传统腹腔镜手术组(55例),对纳入患者的围手术期指标进行对比,以P<0.05为差异有统计学意义。结果对于宫颈癌及子宫内膜癌患者,相比于传统腹腔镜手术组,机器人手术组的淋巴结清扫数目多,术中出血量少,胃肠道功能恢复时间短,但手术时间长(P<0.05)。2组患者术后住院时间及引流管留置时间比较,差异无统计学意义(P>0.05)。结论达芬奇机器人辅助腹腔镜手术在妇科恶性肿瘤治疗时较传统腹腔镜具有视野清晰、淋巴结清扫数目多、术中出血量少、胃肠道功能恢复快等优势,是一种安全可行的手术方式。Objective To explore the feasibility and safety of Da Vinci robotic-assisted laparoscopic surgery for patients with gynecological malignant tumors,and to provide a basis for determining the surgical methods in clinical practice.Methods 107 patients with gynecological malignant tumors were retrospective analyzed and were divided into robotic surgery group(52 cases)and traditional laparoscopic surgery group(55 cases)according to different surgical methods.Comparative statistical analysis of perioperative indicators of all patients,and the difference was statistically significant with(P<0.05).Results Among patients with endometrial cancer,compared with traditional laparoscopic surgery,the number of lymph node dissection in the robotic surgery group was large,the amount of intraoperative bleeding was small,and the recovery time of gastrointestinal function was short,but the operation time was long(P<0.05).There was no statistically significant difference between the two groups in the postoperative hospitalization time and drainage tube retention time(P>0.05).Conclusion Da Vinci robotic-assisted laparoscopic surgery is a safe and feasible surgical method in the treatment of patients with gynecological malignant tumors,which has the advantages of large number of lymph node dissection,less intraoperative bleeding and short recovery time of gastrointestinal function.
