经脐单孔腹腔镜手术治疗盆腔脓肿的可行性及有效性评价

摘　　要：目的分析经脐单孔腹腔镜在盆腔脓肿治疗中的可行性及有效性,为盆腔脓肿腹腔镜手术方式的选择提供参考。方法选取因盆腔脓肿行腹腔镜手术治疗的患者103例,根据腹腔镜手术治疗入路的不同将其分为2组,A组(经脐单孔腹腔镜组)41例、B组(传统多孔腹腔镜组)62例。结果经脐单孔腹腔镜组较传统多孔腹腔镜组术中出血量、术后WBC恢复时间、术后下地活动时间、胃肠功能恢复时间、术后疼痛恢复时间及术后住院时间比较,经脐单孔腹腔镜组较传统多孔腹腔镜组临床效果好(P<0.05)。而手术时间、术后体温恢复时间、术后引流管放置时间、术后抗生素应用时间、次均住院费、术后邻近器官损伤、术后肠梗阻及其他并发症等指标,2组患者比较,差异无统计学意义(P>0.05)。结论经脐单孔腹腔镜手术治疗盆腔脓肿具有较好的可行性、安全性和有效性。Objective To explore the feasibility and effectiveness of transumbilical single-port laparoscopic surgery in the treatment of pelvic abscess,and provide reference for the selection of laparoscopic surgery for pelvic abscess.Methods 103 patients accepted laparoscopic surgery for pelvic abscess were selected and divided into two groups according to the different approaches of laparoscopic surgery,wherein group A(41 cases in transumbilical single-port laparoscopic group)and group B(62 cases in traditional multi-port laparoscopic group).Results Compared with the traditional multi-port laparoscopic group,the transumbilical single-port laparoscopic group had better clinical effects in intraoperative blood,postoperative WBC recovery time,postoperative ground activity time,gastrointestinal function recovery time,postoperative pain recovery time and postoperative hospital stay time(P<0.05).There was no statistically significant difference between the two groups in the operation time,postoperative temperature recovery time,postoperative drainage tube placement time,postoperative antibiotic application time,average hospitalization costs.There was no statistical difference in postoperative adjacent organ injury,the incidence of intestinal obstruction and other complicationsbetween the two groups transumbilical single-port laparoscopic group and traditional multi-port laparoscopic group(P>0.05).Conclusion Transumbilical single-port laparoscopic operation for pelvic abscess has good feasibility,safety and efficacy.
