摘 要:目的比较帐篷钉技术与传统引导骨再生术(GBR)治疗牙槽骨缺损的效果。方法选取牙槽嵴吸收明显、种植修复前需先行骨增量术的患者22例,按简单随机分组法,分为帐篷钉技术组(试验组)11例和传统GBR组(对照组)11例,并开展骨增量手术,采用CBCT测量术区骨量、骨质变化,比较两组成骨效果。结果试验组前牙区水平向骨增量与对照组比较,差异无统计学意义(P>0.05);总体垂直向骨增量显著高于对照组(P<0.05);后牙区水平向骨增量显著高于对照组(P<0.05);两组间垂直向骨增量比较,差异无统计学意义(P>0.05);总体水平骨增量与垂直骨增量,试验组均显著高于对照组(P>0.05)。结论应用帐篷钉技术有利于提高骨增量效果,且预后骨质良好,成骨效果稳定,支抗钉可作为帐篷钉的选择之一。Objective To compare the effect of tent nail technique and conventional guided bone regeneration(GBR)in the treatment of alveolar bone defects.Methods Twenty-two patients with significant alveolar ridge resorption and requiring bone augmentation prior to implant restoration were selected.The patients were divided into a tent nail technique group and a conventional GBR group according to a simple randomized grouping method.The bone augmentation surgery was performed.Cone-beam computed tomography(CBCT)was used to measure bone mass and bone quality changes in the operation area.The osteogenic effect was compared between the two groups.Results There was no statistical difference in horizontal bone gain between the two groups(P>0.05).The vertical bone gain in the tent nail group was significantly higher than that in the control group(P<0.05).The horizontal bone increment of the tent nail group in the posterior teeth area was significantly higher than that of the control group(P<0.05).There was no statistical difference in vertical bone gain between the two groups(P>0.05).The overall horizontal bone gain and vertical bone gain in the tent nail group were significantly higher than that in the conventional GBR group(P<0.05).Conclusions The application of tent nail technique is beneficial to improve the bone augmentation effect with good prognosis and stable osteogenesis.Strut-resistant nail can be used as one of the options for tent nails.
