妇科恶性肿瘤术后肢体淋巴水肿患者自我隐瞒现状及影响因素分析

摘　　要：目的调查妇科恶性肿瘤术后肢体淋巴水肿患者自我隐瞒现状及其影响因素。方法2022年3~7月采用一般资料问卷、自我隐瞒量表、心理痛苦温度计、自尊量表与领悟社会支持量表对成都市2所三甲医院淋巴水肿康复门诊收治的270例妇科恶性肿瘤术后肢体淋巴水肿患者进行调查。结果妇科恶性肿瘤术后肢体淋巴水肿患者自我隐瞒量表得分为(39.00±4.11)分,高自我隐瞒水平190例(70.4%)。多元线性回归分析显示,年龄、术后是否放化疗、经济状况、日常生活活动能力、自尊、心理痛苦和领悟社会支持是自我隐瞒的影响因素(P<0.05)。结论妇科恶性肿瘤术后肢体淋巴水肿患者自我隐瞒水平较高,提示临床护士应该以患者自我隐瞒的影响因素为切入点,实施针对性干预。Objective To investigate the current status of self-concealment of patients with lymphedema after gynecological malignant tumor surgery and its influencing factors.Methods From March 2022 to July 2022,270 patients with lymphedema after gynecological malignant tumor surgery treated at lymphedema outpatient departments of two tertiary hospitals in Chengdu were selected.The investigation was performed by using general information questionnaire,self-concealment scale,psychological distress thermometer and Perceived Social Support Scale.Results The score of self-concealment scale of these patients was 39.00+4.11,and 190 patients(70.4%)had a high level of self-concealment.Multiple linear regression analysis showed that age,postoperative chemoradiotherapy,economy,daily life activity,self-esteem,psychological distress and understanding social support were influencing factors of self-concealment(P<0.05).Conclusions Patients with lymphedema after gynecological malignant tumor surgery have a high level of self-concealment.This suggests that clinical nurses should take the factors affecting patients′self-concealment as an entry point and implement targeted interventions.
