胃瘫方烫熨结合脐灸促进胃部肿瘤术后胃瘫恢复的效果观察

摘　　要：目的观察自拟胃瘫方烫熨结合隔药脐灸法治疗胃部肿瘤术后胃肌轻瘫的临床效果。方法纳入我院符合标准的102例胃部肿瘤术后胃瘫患者为研究对象,采用随机分组法分为观察组与对照组,每组各51例。对照组采取常规基础疗法,观察组增加胃瘫方烫熨结合隔药脐灸法方案干预。对比两组患者促胃液素水平、胃瘫主要症状指数量表得分及临床疗效的差异。结果①两组胃瘫患者治疗前的促胃液素水平及胃液引流量无明显差异(P>0.05),治疗后两组患者上述指标均有改善(P<0.05),但观察组患者促胃液素水平显著高于对照组,胃液引流量显著低于对照组(P<0.05);②治疗前两组患者胃排空时间无明显差异(P>0.05),治疗后两组胃排空时间均明显缩短(P<0.05),且观察组胃排空时间显著短于对照组,胃管拔除时间与术后首次进食时间均明显早于对照组(P<0.05);③观察组患者临床总有效率显著高于对照组(P<0.05)。结论自拟胃瘫方烫熨结合隔药脐灸法可改善胃部肿瘤术后胃瘫综合征患者的胃瘫恢复效果,促进促胃液素分泌,提高临床疗效。Objective To observe the clinical effects of self-prepared gastroparesis recipe ironing therapy plus moxibustion on the umbilicus with separated medicine on gastroparesis after gastric tumor operation.Methods A total of 102 patients with gastroparesis after gastric tumor operation were enrolled in our hospital.They were randomly divided into observation group and control group with 51 patients in each group.The control group was treated with routine basic therapy,while the observation group was added with the self-prepared gastroparesis recipe ironing therapy and medicine-separated moxibustion on the umbilicus.Compare the differences of gastrin level,scores of gastroparesis cardinal symptom index,and clinical efficacy between the two groups.Results①There was no significant difference in gastrin level and gastric juice drainage between the two groups before treatment(P>0.05).After treatment,both groups had improvement in the above two indexes,but the level of gastrin in the observation group was higher than that in the control group,and the gastric drainage was lower than that in the control group(P<0.05).②There was no significant difference in the time for gastric emptying before treatment(P>0.05).After treatment,both groups had the gastric emptying time decreased(P<0.05),but the time for gastric emptying in the observation group was shorter than in the control group,and the time of gastric tube extraction and first eating after operation were earlier than in the control group(P<0.05);③The total clinical effective rate in the observation group was higher than in the control group(P<0.05).Conclusion The self-made gastroparesis prescription ironing therapy combined with medicine-separated moxibustion on umbilicus can improve the recovery of gastroparesis in patients with gastroparesis syndrome after gastric tumor operation,and promote the secretion of gastrin and optimize the clinical effect.
