摘  要:目的 探讨健康成人远端复合肌肉动作电位时限(DCMAPD)及时间离散度(TD)的定量检测,为周围神经脱髓鞘病变诊断提供依据。方法 选取120例健康志愿者,其中男56例,女64例,年龄20~84岁。将其按年龄20~29岁、30~39岁、40~49岁、50~59岁、60~69岁及>70岁分为6组。依次对所有志愿者的胫神经、腓总神经、正中神经、尺神经进行常规运动神经传导检测,分析不同年龄段和不同性别间远端复合肌肉动作电位时限(DCMAPD)和时间离散度(TD)的差异。结果 四条神经中仅腓总神经DCMAPD在不同年龄组差异具有统计学意义(P<0.05),其余三条神经DCMAPD在不同年龄组间差异均无统计学意义(P>0.05);正中神经、尺神经、胫神经和腓总神经DCMAPD和TD在不同性别间差异均无统计学意义(P>0.05)。结论健康成人DCMAPD值在正中神经、尺神经、胫神经中可不分年龄及性别采用统一的参考值,而腓总神经则按照50岁前后来区分参考值比较合理,TD值可不分年龄及性别采用统一参考值。Objective To explore the quantitative detection of distal complex muscle action potential duration(DCMAPD) and temporal dispersion(TD) in healthy adults, and to provide the basis for the diagnosis of peripheral nerve demyelination diseases. Methods A total of 120 healthy adult volunteers who met the inclusion criteria were enrolled in the study, including 56 males and 64 females, aged 20-84 years old. They were divided into 6 groups according to the age of 20-29 years old, 30-39 years old, 40-49 years old, 50-59 years old, 60-69 years old and >70 years old. Routine motor nerve conduction detection were performed on the tibial nerve, common peroneal nerve, median nerve and ulnar nerve of all volunteers in turn, and the differences of DCMAPD and TD between different ages and different genders were analyzed. Results Among the four nerves, only the common peroneal nerve had a statistically significant difference for DCMAPD among different age groups(P<0.05), while the other three nerves had no statistically significant difference among different age groups(P>0.05). There was no significant difference in DCMAPD and TD of median nerve, ulnar nerve, tibial nerve and common peroneal nerve between different genders(P>0.05). Conclusion The DCMAPD value of healthy adults in the median nerve, ulnar nerve and tibial nerve can use a unified reference value regardless of age and sex, while the common peroneal nerve is more reasonable to use a reference value before and after 50 years old, and the TD value can use a unified reference value regardless of age and sex.
