摘 要:目的 探讨脑卒中磁敏感加权成像(SWI)上突出血管征(PVS)与软脑膜侧支循环的相关性。方法 回顾性分析单侧大脑中动脉M1段闭塞急性缺血性脑卒中患者的临床及影像资料。采用区域软脑膜侧支评分(rLMC)评估CTA上软脑膜侧支,使用改良berta卒中项目早期CT评分(ASPECTS)评估SWI上PVS的程度。通过Spearman相关分析筛选与rLMC评分相关的临床及影像学变量,应用多重线性回归进一步分析rLMC评分相关的因素。结果 纳入44例患者,男37例,女7例,平均年龄(51.27±10.06)岁。Spearman相关分析显示rLMC评分与改良SWI-ASEPCTS、入院NIHS.S评分、空腹血糖、糖化血红蛋白呈负相关(r=-0.471.,P=0.001;r=-0.386,P=0.010;r=-0.367,P=0.014;r=-0.414,P=0.005);多重线性回归分析显示,改良SWIASPECTS与rLMC评分存在独立相关。结论SWI上PVS与软脑膜侧支呈中度负相关,在一定程度上反映软脑膜侧支循环的代偿水平。Objective To explore the relationship between prominent vessel sign(PVS)on susceptibility weighted imaging(SWI)and leptomeningeal collateralization.Methods Clinical and imaging data of acute ischemic stroke patients with unilateral middle cerebral artery M1 segment occlusion were retrospectively reviewed.Regional leptomeningeal score(rLMC)was used to evaluate the leptomeningeal collateralization on CTA.Use the modified Alberta Stroke Program Early CT Score(ASPECTS)to assess the extent of PVS on SWI.Spearman correlation analysis was performed to explore the correlation of rLMC scores with clinical and radiographic variables.Multiple linear regression was used to further analyze the putative related factors of rLMC score.Results Forty-four patients were included,including 37 males and 7 females,with an average age of(51.27±10.06)years.Spearman correlation analysis showed that rLCM score was negatively correlated with the modified SWIASEPCTS,NIHSS score at admission,fasting blood glucose and glycosylated hemoglobin(r=-0.471,P=0.001;r=-0.386,P=0.010;r=-0.367,P=0.014;r=-0.414,P=0.005).Multilinear regression analysis showed that the modified SWI-ASPECTS was independently associated with rLMC score.Conclusion The PVS on SWI is moderately negatively correlated with leptomeningeal collateralization,which reflects the compensatory level of leptomeningeal collateralization to some extent.
