新型OTSC吻合夹止血术治疗溃疡性上消化急性出血的疗效及并发症分析

摘　　要：目的分析新型OTSC吻合夹止血术治疗溃疡性上消化急性出血的疗效及并发症。方法选择2019年1月-2021年1月经徐州医科大学附属淮海医院收治的溃疡性上消化急性出血患者113例,根据患者意愿将其分为观察组(n=63)与对照组(n=50),观察组行新型OTSC吻合夹止血术治疗,对照组行传统TTSC内镜夹止血术治疗。收集患者一般资料:性别、平均年龄、出血部位、Forrest分级、平均Rockall评分。比较两组患者临床疗效、临床指标:患者输血量、止血时间、住院时间、术后再出血率、并发症等。结果观察组患者有效率高于对照组,差异有统计学意义(P<0.05)。观察组患者输血量、止血时间、住院时间、术后再出血率均低于对照组患者,且止血成功率高于对照组,差异有统计学意义(P<0.05)。两组患者治疗后均未出现消化道瘘、穿孔等严重并发症。结论新型OTSC吻合夹止血术可提高溃疡性上消化急性出血患者临床疗效,且安全性较高。Objective To analyze the efficacy and complications of new over-the-scope-clip(OTSC)hemostasis in treatment of acute hemorrhage of ulcerative upper digestive tract.Methods From January 2019 to January 2021,113 patients with acute hemorrhage of ulcerative upper digestive tract admitted to Huaihai Hospital Affiliated to Xuzhou Medical University were selected and divided into observation group(n=63)and control group(n=50)according to their wishes.The observation group was treated with OTSC hemostasis,while the control group was treated with traditional through-the-scope-clip(TTSC)hemostasis.General data of patients were collected,including gender,average age,bleeding site,Forrest classification,and average Rockall score.The clinical efficacy and clinical indicators of the two groups were compared including blood transfusion volume,hemostasis time,hospitalization time,postoperative rebleeding rate,and complications.Results The effective rate of the observation group was higher than that of the control group(P<0.05).The blood transfusion volume,hemostasis time,hospitalization time,and postoperative rebleeding rate in the observation group were lower than those in the control group,and the success rate of hemostasis was higher than that in the control group(P<0.05).No serious complications such as digestive tract fistula and perforation occurred in both groups after treatment.Conclusion The new OTSC hemostasis can improve the clinical efficacy of patients with acute hemorrhage of the ulcerative upper digestive tract,and has high safety.
