一站式CT对椎基底动脉扩张延长症患者后循环血流灌注的研究

摘　　要：目的探讨一站式颅脑CT评估椎基底动脉扩张延长症(VBD)患者后循环血流灌注状态的应用价值.方法选取符合VBD诊断标准的患者49例(实验组)与正常对照22例(对照组).在后处理工作站利用全脑灌注分析软件生成4D灌注参数图评估后循环区域的血流灌注状态,主要灌注参数包括局部脑血流量(rCBF)、局部脑血容量(rCBV)、平均通过时间(MTT)、达峰时间(TTP).结果全脑脑灌注分析显示,与对照组相比,实验组双侧枕叶、脑桥、小脑半球TTP延长,差异有统计学意义(P<0.05),左侧小脑半球、右侧脑桥MTT较对照组延长,而2组rCBF、rCBV无明显差异(P>0.05).结论一站式颅脑CT可准确评估VBD患者后循环血流灌注状态.Objective To investigate the application value of one-stop brain CT to assess the perfusion state of posterior circulation in patients with vertebrobasilar dolichoectasia(VBD).Methods Forty-nine patients(experimental group),and twenty-two cases of normal patients(control group)who meet the diagnostic criteria of VBD were selected.The 4D perfusion parameter map was generated by the whole brain perfusion analysis software on the post-processing workstation to evaluate the perfusion state of the posterior circulation.The main perfusion parameters included regional cerebral blood flow(rCBF),regional cerebral blood volume(rCBV),mean transit time(MTT)and time to peak(TTP).Results The analysis of whole brain perfusion showed that TTP in bilateral occipital lobe,pon and cerebellar hemisphere was increased than that of the control group,and the difference was statistically significant(P<0.05),MTT of left cerebellar hemisphere and right pon was increased than that of the control group,but there was no significant difference in rCBF and rCBV between the two groups(P>0.05).Conclusion One-stop brain CT can accurately evaluate the perfusion state of posterior circulation inpatients with VBD.
