摘  要:目的比较和观察临床中穿髓型楔状缺损的前磨牙,在完善根管治疗后分别采用玻璃纤维桩辅助固位和流动树脂加纳米树脂充填,再进行计算机辅助设计制作(CAD/CAM)铸瓷全冠的修复效果。方法选择北京市某综合医院口腔科修复的196颗穿髓型楔状缺损前磨牙,随机分为研究组和对照组,其中研究组93例96颗,对照组94例100颗。2组进行完善根管治疗后,研究组采用流动树脂加纳米树脂充填,对照组采用玻璃纤维桩及双固化树脂充填,最后均进行CAD/CAM铸瓷全冠修复。参考改良美国公共健康协会修订标准(USPHS),分别在修复后半年、1年、2年和3年进行复诊检查和评价效果,综合比较两种方法的临床修复效果。结果根据改良USPHS评价标准,各复查点2组修复体A级评价百分比都>80%,A级+B级评价和为100%,2组临床评价指标差异无统计学意义(P>0.05)。结论在穿髓型楔状缺损的前磨牙铸瓷全冠修复前,可以酌情选择流动树脂加纳米树脂作为充填方案之一,但其长期效果仍需进一步观察。Objective To compare the repair effect on premolars with pulp-penetrating wedge-shaped defect that were managed by root canal treatment,followed by glass fiber post-assisted retention versus flow resin plus nano-resin filling,and then using computer-aided design CAD/CAM ceramic crown retention.Methods A total of 196 premolars with pulp-penetrating wedge-shaped defect repaired by the Department of Stomatology of a general hospital in Beijing were selected and randomly divided into experimental group(93 patients,96 teeth)and control group(94 patients,100 teeth).After complete root canal treatment in the two groups,the experimental group was filled with flow resin plus nano-resin filling,and the control group was filled with glass fiber post and dual-curing resin.CAD/CAM ceramic crown retention was finally performed in both groups.Referring to the modified US Public Health Service(USPHS),follow-up examination and evaluation were conducted in six months,1 year,2 years and 3 years after repair,and the clinical repair effects of the two methods were comprehensively compared.Results According to the modified USPHS evaluation standard,the percentage of the two groups of restorations at each follow-up time point was above 80%,and the sum of the grades A and B was 100%.There were no significant differences in the clinical evaluation indicators between the two groups(P>0.05).Conclusion Flow resin plus nano-resin can be selected as one of the filling options as appropriate before CAD/CAM ceramic crown retention to pulp-penetrating penetrating wedge-shaped defects,but its long-term effect still needs further observation.
