摘 要:目的分析以问题为基础的教学法(problem-based learning,PBL)联合以病例为基础的教学法(case-based learning,CBL)应用于骨科实习中的价值。方法于滨州医学院附属医院骨科2019年6月—2021年7月间选取82名实习生作为研究对象。根据区组化随机数分组法将其分为实践组(n=41,PBL联合CBL教学法)和参照组(n=41,采用传统教学)。对两组考核成绩、实习积极性、问题分析与解决能力、实习评价调查结果进行评价。结果实践组实习生病例分析、临床实践及理论成绩均高于参照组,且实习积极性较参照组也明显更高,差异有统计学意义(P<0.05);实习前两组问题分析与解决能力评分结果差异无统计学意义(P>0.05);而实习后实践组的查找循证、问题分析、提出对策及问题解决能力评分均较参照组更高,差异有统计学意义(P<0.05);实践组对实习教学模式能提高学习兴趣、动手能力、自学能力、临床思维能力及应变能力的认可度评价均高于参照组,差异有统计学意义(P<0.05)。结论将PBL联合CBL教学法应用于骨科实习中,有利于提高实习生的积极性、考核成绩、问题分析与解决能力,可有效提高实习评价效果。Objective To analyze the value of problem-based(PBL)combined case-based(CBL)teaching method in orthopedic practice.Methods A total of 82 interns were selected from the department of orthopedics of Affiliated Hospital of Binzhou Medical College from June 2019 to July 2021.According to the block random number grouping method,they were divided into practice group(n=41,using problem-based combined case-based teaching)and reference group(n=41,using traditional teaching).The assessment results,practice enthusiasm,problem analysis and solving ability and practice evaluation survey results of the two groups were evaluated.Results The results of case analysis,clinical practice and theory of interns in the practice group were higher than those in the reference group,and their practice enthusiasm were significantly higher than that in the reference group(P<0.05).There was no significant difference between the two groups before practice(P>0.05).The scores of evidence-based search,problem analysis,countermeasures and problem-solving ability in the practice group were higher than those in the reference group,the differences were statistically significant(P<0.05).The recognition evaluation of the practice group on the improvement of learning interest,practical ability,self-study ability,clinical thinking ability and adaptability of the practice teaching mode was higher than that of the reference group,the differences were statistically significant(P<0.05).Conclusion The application of problem-based combined with case-based teaching in orthopedic practice is conducive to improve the enthusiasm,assessment results,problem analysis and solution ability of interns,and can effectively improve the effect of practice evaluation.
