急性缺血性卒中高龄患者静脉溶栓治疗后早期神经功能恶化因素分析

摘　　要：目的:探讨急性缺血性卒中(Acute ischemic stroke,AIS)高龄患者)静脉溶栓后早期神经功能恶化(early neurological deterioration,END)的因素分析。方法:收集高龄患者溶栓病人资料,同时纳入溶栓后导致END危险因素进行多因素Logistic回归分析。结果:本研究纳入高龄AIS 62例,男性28例,女性34例,平均年龄(85.0±4.1)岁,其中发生END组20例;溶栓前和溶栓后发生END组血糖水平明显高于非END组(P<0.05),将存在统计学差异的因素作为自变量进行多因素的Logistic逐步回归分析发现,溶栓后空腹血糖[OR95%CI=(1.091,2.857)是AIS溶栓后发生END独立危险因素。结论:高龄患者溶栓后AIS空腹血糖水平是发生END的独立危险因素。Objective:To investigate the factors of neurological deterioration(END)in the early stage of advanced Acute ischemic stroke(AIS)thrombolysis.Methods:The data of thrombolytic patients in elderly patients were collected,and the risk factors for END after thrombolysis were also included for multifactor logistic regression analysis.Results:In this study,62 cases of advanced AGE AIS,28 cases of male and 34 cases of female,with an average age(85.0±4.1)years old were included,of which 20 cases occurred in the END group.The blood glucose levels of the END group after thrombolysis were significantly higher than those in the non-END group(P<0.05).A multivariate logistic stepwise regression analysis using statistically different factors as independent variables found that fasting blood glucose after thrombolysis[OR95%CI=(1.091,2.857)was an independent risk factor for END after AIS thrombolysis.Conclusion:AIS fasting blood glucose levels after thrombolysis in elderly patients are independent risk factors for the development of END.
