钝性分离联合透明质酸钠复合溶液治疗泪沟畸形疗效观察

摘　　要：目的观察钝性分离联合透明质酸钠复合溶液填充改善泪沟畸形的临床疗效和安全性。方法共纳入23例泪沟畸形患者,使用钝针广泛分离泪沟区真皮深层,填充注射透明质酸钠复合溶液,拟通过创伤-修复-填充-刺激再生的模式改善泪沟畸形。由专业医师根据泪沟畸形评分量表给予评分结果,患者依据个体主观满意度给予评分判定治疗效果。结果共23例患者完成了治疗和随访。治疗完成3~6个月内可保持最佳治疗效果,随着时间的增加治疗效果有不同程度下降,但在12个月内仍存在一定治疗效果。治疗前泪沟畸形程度评分为(5.76±1.45)分,治疗后即刻评分降至(2.57±1.34)分,治疗3个月、6个月、12个月评分分别为(2.78±1.41)分、(3.04±1.36)分、(4.11±1.13)分,治疗前后评分差异均有统计学意义;治疗即刻全部患者达到满意的治疗效果,治疗完成6个月后,21例(91.30%)患者表示对治疗效果满意;治疗完成12个月后,仍有17例(73.91%)患者表示对治疗效果满意。结论钝性分离联合透明质酸钠复合溶液填充泪沟,可明显改善泪沟畸形,效果确切,疗效显著、持久。Objective To observe the clinical efficacy and safety of blunt dissection combined with sodium hyaluronate compound solution for the treatment of tear trough deformity.Methods Select 23 tear trough deformity patients, using a blunt needle to separate the deep layer of the dermis in the tear trough area, filling with sodium hyaluronate compound solution, intended to treat the tear trough deformity through wound-repair-fill-stimulate regeneration.Professional doctors scored the results according to the tear trough deformity clinical evaluation scale(TTRS), and patients scored according to individual subjective satisfaction to judge the treatment effect.Results All 23 patients completed the treatment and follow-up.The best therapeutic effect can be maintained within 3 to 6 months after the treatment, and with the extension of time, the therapeutic effect decreased to different degrees. There is still a certain therapeutic effect within 12 months.Before treatment, the score of tear trough deformity was(5.76±1.45) points, and the score decreased to(2.57±1.34) points immediately after treatment, and the scores were(2.78±1.41) points,(3.04±1.36) points,(4.11±1.13) points, the difference of the scores before and after treatment was statistically significant. All patients achieved satisfactory therapeutic effect immediately after treatment.After 6 months 21 cases(91.30%) of patients expressed that they were satisfied with the therapeutic effect, 17 cases(73.91%) of patients still expressed satisfaction with the treatment effect after 12 months of treatment.Conclusion Blunt dissection combined with sodium hyaluronate compound solution can significantly improve the problem of tear trough deformity, and the effect is significant and long-lasting.
