Nd:YAG激光联合Gluma脱敏剂治疗非龋性牙颈部缺损引起牙本质过敏症的效果观察

摘　　要：目的:观察Nd:YAG激光联合Gluma脱敏剂治疗非龋性牙颈部缺损引起的牙本质过敏症的效果。方法:选取2021年5月—2022年8月南京医科大学附属无锡市第二人民医院口腔科收治的非龋性牙颈部缺损引起的牙本质过敏症(DH)患者43例,患牙101颗,按照单盲法随机分为G1组和G2组两组。G1组给予Gluma脱敏剂脱敏治疗(患者20例,患牙49颗),G2组采取Nd:YAG激光联合Gluma脱敏剂脱敏治疗(患者23例,患牙52颗)。治疗前、治疗后即刻、治疗后1个月、治疗后3个月分别采用视觉模拟评分法(VAS)对患牙进行温度及机械性刺激反应的测试,并记录结果。结果:两组治疗后不同时期VAS分值较治疗前明显降低,差异有统计学意义(P<0.01),G2组治疗后即刻温度刺激VAS分值略低于G1组,差异无统计学意义(P>0.05);机械刺激VAS评分G2组低于G1组,差异有统计学意义(P<0.05);治疗后1个月及3个月,温度及机械刺激G2组VAS评分低于G1组,差异有统计学意义(P<0.05)。结论:Nd:YAG激光联合Gluma脱敏剂治疗非龋性牙颈部缺损引起的牙本质过敏症与单一应用Gluma脱敏剂相比,具有更持久稳定的效果,可在临床应用。Objective To evaluate the effect of Nd:YAG laser combined with Gluma desensitizer on dentin hypersensitivity caused by non-carious cervical lesions.Methods 43 patients with dentin hypersensitivity caused by non-carious cervical lesions and unsuitable for resin filling were included.101 teeth were randomly divided into two groups:G1 group(20 patients,49 teeth)treated with Gluma desensitizer and G2 group(23 patients,52 teeth)treated with Nd:YAG laser combined with Gluma desensitizer.Visual analogue scale(VAS)was used to test the temperature and mechanical stimulation responses of the teeth before,immediately after,one month and three months after treatment,and the results were recorded.Results The VAS of the two groups at different periods after treatment were significantly lower than those before treatment(P<0.01).The VAS of temperature stimulation in group G2 was slightly lower than that in group G1 immediately after treatment,but the difference was not statistically significant(P>0.05).The VAS of mechanical stimulation in group G2 was lower than that in group G1,the difference was statistically significant(P<0.05).One month and three months after treatment,the temperature and mechanical stimulation in group G2 were significantly better than those group G1,and the difference was statistically significant(P<0.05).Conclusion Nd:YAG laser combined with Gluma desensitizer is more effective than Gluma desensitizer alone in the treatment of dentin hypersensitivity caused by non-carious cervical lesions.
