三阴性乳腺癌空窗期的中医体质特征及与预后相关性分析

摘　　要：目的:分析三阴性乳腺癌(TNBC)空窗期的中医体质特征及其与预后的相关性,以期为TNBC空窗期的中医药防治提供临床参考。方法:选取2018年3月—2021年3月福建医科大学附属第二医院收治的已完成规范的术后辅助治疗的TNBC患者120例,收集其相关的基本资料及病例资料,判定中医体质类型,研究TNBC空窗期患者的中医体质特征与临床病理、及复发转移等的相关性。结果:TNBC空窗期患者多为偏颇体质,其中前三者分别为气虚、气郁、阳虚。TNBC患者的中医体质情况与年龄分布比较,差异有统计学意义(P<0.05);TNBC患者中医体质的Ki。67分布情况比较,差异无统计学意义(P>0.05);TNBC患者中医体质的肿瘤大小比较,差异有统计学意义(P<0.05);TNBC患者中医体质的组织学分型比较,差异无统计学意义(P>0.05);TNBC患者中医体质的TNM分期比较,差异有统计学意义(P<0.05)。TNBC患者中医体质的BRCA1情况分布比较,差异有统计学意义(P<0.05)。TNBC患者的虚性体质与复发转移为正相关关系(P<0.05),TNBC患者的实性体质与复发转移无相关性(P>0.05)。结论:中医体质可以成为TNBC空窗期患者判断预后的指征之一,以此为基础的中医辨证论治可以作为TNBC空窗期患者的临床干预手段,改善其预后。Objective To study the characteristics of traditional Chinese medicine constitution and prognosis of triple-negative breast carcinoma patients during the empty window period,so as to provide a clinical reference for the prevention and treatment of triple-negative breast carcinoma.Methods We collected 120 patients with triple-negative breast carcinoma who had been confirmed by surgery and pathology,and had completed the standardized postoperative adjuvant therapy from March 2018 to March 2021,to study the correlation between TCM Constitution and clinical pathology,recurrence and metastasis of triple-negative breast carcinoma.Results Triple-negative breast carcinoma was mostly biased constitution,and the first three were Qi deficiency,qi depression and yang deficiency.There was significant difference in TCM constitution and age distribution between triple-negative breast carcinoma(P<0.05).Compared with Ki-67 and organizational credit type,there was no significant difference(P>0.05);Compared with tumor size and TNM stage composition,the difference was statistically significant(P<0.05);Compared with the composition of recurrence and metastasis,there was no significant difference(P>0.05).The distribution of BRCA1 in TCM constitution of triple-negative breast carcinoma patients was statistically significant(P<0.05).There was a positive correlation between the virtual constitution and recurrence and metastasis of patients with triple-negative breast cancer(P<0.05).There was no significant difference between solid constitution and recurrence and metastasis(P>0.05).Conclusion TCM Constitution can be one of the indications for prognosis of triple-negative breast carcinoma.Based on this,TCM syndrome differentiation and treatment can serve as a clinical intervention for triple-negative breast carcinoma and improve prognosis.
