摘　　要：目的 分析神经导航引导联合神经内镜经鼻蝶切除垂体腺瘤的临床疗效。方法 选取2020-01—2022-01在惠州市中心人民医院住院的100例垂体腺瘤患者为研究对象,随机分为观察组(n=50,神经导航引导联合神经内镜经鼻蝶切除垂体腺瘤手术)与对照组(n=50,神经内镜经鼻蝶切除垂体腺瘤手术治疗),均术后跟踪随访观察半年。比较2组患者手术有效性指标、内分泌激素水平及并发症发生情况之间的差异。结果 观察组患者肿瘤全切率为98.00%,视力改善率为92.00%,均高于对照组的86.00%、78.00%;肿瘤复发率为0,低于对照组的8.00%;治疗后内分泌激素hGH、ACTH、PRL水平均低于对照组,TSH水平高于对照组;并发症发生率4.00%,低于对照组的16.00%,差异均有统计学意义(P<0.05)。结论 神经导航引导联合神经内镜经鼻蝶切除垂体腺瘤能够提高肿瘤全切与视力改善率,有效降低并发症发生率与肿瘤复发率,改善内分泌激素水平,临床疗效与安全性均更加显著。Objective To explore the clinical efficacy of neuronavigation-guided therapy combined with neuroendoscopic resection of pituitary adenoma.Methods From January 2020 to January 2022,100 patients with pituitary adenoma hospitalized in Huizhou Central People’s Hospital were selected as the study subjects and di⁃vided into observation group(n=50,neuroendoscopic transnasal butterfly surgery)and control group(n=50,neuro⁃navigation-guided neuroendoscopic transnasal butterfly surgery).Both groups were followed up for half a year.The differences in surgical effectiveness index,endocrine hormone levels and complications were compared between the two groups.Results In the observation group,the total tumor resection rate of 98.00%and 92.00%visual acuity improvement rate were all higher than 86.00%and 78.00%in the control group,and the tumor recurrence rate of 0 was lower than 8.00%in the control group,the treated endocrine hormones hGH,ACTH,and PRL levels were lower than the control group,TSH levels were higher than the complication rate of 4.00%was lower than 16.00%of the control group,which were statistically significant(P<0.05).Conclusion Neuronavigation guidance combined with neuroendoscopic transnasal butterfly resection of pituitary adenoma can improve the rate of total tumor resection and vision improvement,effectively reduce the incidence rate of complications and tumor re⁃currence,improve the level of endocrine hormone,clinical efficacy and safety are more significant.
