翼状胬肉切除联合改良结膜移植术、角膜缘干细胞移植术治疗原发性翼状胬肉的效果比较

摘　　要：目的比较翼状胬肉切除联合改良结膜移植术、角膜缘干细胞移植术治疗原发性翼状胬肉的效果。方法选取2018年1月至2021年2月于我院行手术治疗的70例(70眼)原发性翼状胬肉患者作为研究对象,根据手术方式不同将其分为A组和B组,各35例(35眼)。A组给予翼状胬肉切除联合改良结膜移植术,B组给予翼状胬肉切除联合角膜缘干细胞移植术。比较两组的治疗效果。结果两组术后1周的角膜及结膜创面均恢复良好。术后2周,A组的泪膜破裂时间(BUT)短于B组(P<0.05);术后2周、1个月,两组的BUT均短于术前(P<0.05);术后3个月,两组的BUT均长于术前(P<0.05)。术后2周,A组的角膜荧光素染色(FL)评分高于B组(P<0.05);术后2周、1个月,两组的FL评分均高于术前(P<0.05)。术后2周,A组的眼表疾病指数(OSDI)评分高于B组(P<0.05);术后2周,两组的OSDI评分高于术前(P<0.05);术后3个月,两组的OSDI评分低于术前(P<0.05)。术后随访6个月,两组均未出现复发及严重并发症。结论翼状胬肉切除联合改良结膜移植术治疗原发性翼状胬肉虽然早期泪膜功能恢复慢,但远期疗效与翼状胬肉切除联合角膜缘干细胞移植术相似。Objective To compare the efficacy of pterygium excision combined with modified conjunctival transplantation and limbal stem cell transplantation in the treatment of primary pterygium.Methods A total of 70 patients(70 eyes)with primary pterygium underwent operative treatment in our hospital from January 2018 to February 2021 were selected as the research objects.According to different operative methods,the patients were divided into group A and group B,with 35 cases(35 eyes)in each group.The group A was given pterygium excision combined with modified conjunctival transplantation,and the group B was given pterygium excision combined with limbal stem cell transplantation.The therapeutic effects of the two groups were compared.Results The corneal and conjunctival wounds of the two groups recovered well at 1 week after operation.At 2 weeks after operation,the tear film break-up time(BUT)of the group A was shorter than that of the group B(P<0.05);at 2 weeks and 1 month after operation,the BUT of the two groups was shorter than that before operation(P<0.05);at 3 months after operation,the BUT of the two groups was longer than that before operation(P<0.05).At 2 weeks after operation,the corneal fluorescein staining(FL)score of the group A was higher than that of the group B(P<0.05);at 2 weeks and 1 month after operation,the FL score of the two groups was higher than that before operation(P<0.05).At 2 weeks after operation,the Ocular Surface Disease Index(OSDI)score of the group A was higher than that of the group B(P<0.05);at 2 weeks after operation,the OSDI score of the two groups was higher than that before operation(P<0.05);at 3 months after operation,the OSDI score of the two groups was lower than that before operation(P<0.05).After 6 months of follow-up,there were no recurrence or serious complications occurred in both groups.Conclusion Although pterygium excision combined with modified conjunctival transplantation in the treatment of primary pterygium has a slow early recover of tear film function,but has similar l
