抑郁症患者口服抗精神病药物不良反应的影响因素分析

摘　　要：目的探讨抑郁症患者口服抗精神病药物产生不良反应的影响因素分析。方法回顾性分析2019年4月至2021年4月进行口服抗精神病药物治疗的抑郁症患者150例,对抑郁症患者口服抗精神病药物治疗发生不良反应原因进行单因素、多因素Logistic回归分析,并根据风险因素进行干预。结果抑郁症患者口服抗精神病药物不良反应累及消化系统、神经系统、循环系统及运动系统;单因素分析表明,抑郁症患者口服抗精神病药物不良反应发生与年龄、给药剂量、过敏史、合并用药、用药疗程以及既往抗精神病药物使用史具有统计意义(P<0.05);以单因素分析中有统计学意义的变量为自变量进行多因素Logistic回归分析,结果表明给药剂量(OR=3.576,95%CI=2.482-6.314)、合并用药(OR=6.411,95%CI=5.682-8.453)、用药疗程(OR=4.081,95%CI=3.235-4.572)、既往抗精神病药物使用史(OR=1.034,95%CI=0.284-4.391)为导致抑郁症患者口服抗精神病药物发生不良反应的高险因素(P<0.05)。结论抑郁症患者口服抗精神病药物不良反应较多,且导致不良反应发生的因素较多,应结合不同患者的特点及危险因素,进行针对性干预,提高抑郁症患者的预后水平。Objective To explore the influencing factors of adverse reactions caused by oral antipsychotic drugs in patients with depression.Methods One hundred and fifty patients with depression who were treated with oral antipsychotic drugs from April 2019 to April 2021 were retrospectively analyzed.The causes of adverse reactions caused by oral antipsychotic drugs in patients with depression were analyzed by univariate and multivariate Logistic regression,and intervention was conducted according to risk factors.Results The adverse reactions of oral antipsychotic drugs in patients with depression involved digestive system,nervous system,circulatory system and motor system.Univariate analysis showed that the incidence of adverse reactions of oral antipsychotic drugs in patients with depression was statistically significant with age,dosage,history of allergy,combined medication,course of medication and previous history of use of antipsychotic drugs(P<0.05).The results of multivariate Logistic regression analysis with the factors in univariate analysis as independent variables showed that the dosage(OR=3.576,95%CI=2.482-6.314),combined medication(OR=6.411,95%CI=5.682-8.453),course of medication(OR=4.081,95%CI=3.235-4.572),past use history of antipsychotic drugs(OR=1.034,95%CI=0.284-4.391)were high risk factors for adverse reactions of oral antipsychotic drugs in patients with depression(P<0.05).Conclusion There are many adverse reactions caused by oral antipsychotic drugs in patients with depression,and there are many factors leading to adverse reactions.Targeted intervention should be carried out according to the characteristics and risk factors of different patients to improve the prognosis of patients with depression.
