针刺联合超声监测胃残余量对重症脑卒中患者肠内营养的疗效影响

摘　　要：目的本研究观察针刺联合超声监测胃残余量对重症脑卒中患者肠内营养的疗效影响。方法选取90例重症脑卒中患者,用随机数字表随机分为3组:传统回抽胃液组、超声测定胃残余量组、针刺联合超声测定胃残余量组。观察并记录营养相关指标、肠内营养并发症及预后指标。结果超声测定胃残余量组和针刺联合超声测定胃残余量组治疗后血清前白蛋白高于传统回抽胃液组(P<0.05),肠内营养达标时间和脱机时间亦缩短(P<0.05)。针刺联合超声测定胃残余量组治疗后血清前白蛋白指标与脱机时间比超声测定胃残余量组更有优势(P<0.05)。另外,两组总体并发症发生率低于传统回抽胃液组(P<0.05)。结论超声测定胃残余量联合针刺可以更好地为重症脑卒中患者提供营养支持,并有助于患者的康复。Objective:To observe the effect of acupuncture combined with ultrasound monitoring of gastric residual volume(GRV)on enteral nutrition(EN)in patients with severe stroke.Methods:A total of 90 patients with severe stroke who were admitted to the intensive care unit were selected and divided into three groups by the random number table method:traditional gastric juice withdrawal group,ultrasound assessment of GRV group,ultrasound assessment of GRV combined with acupuncture group.Nutrition related indexes,EN complications and prognosis were observed and recorded.Results:The serum prealbumin in the ultrasound assessment of GRV group and the ultrasound assessment of GRV combined with acupuncture group was higher than that in the traditional gastric juice withdrawal group(P<0.05),and the time of reaching the standard of EN and offline extubation were also shortened(P<0.05).The indexes of serum prealbumin and the time of offline extubation in the ultra⁃sound assessment of GRV combined with acupuncture group were better than those in the ultrasound assessment of GRV group(P<0.05).In addition,the overall complication rate of these two groups was less than that of the tra⁃ditional gastric juice withdrawal group(P<0.05).Conclusion:Ultrasound assessment of GRV combined with acu⁃puncture can better provide nutritional support for patients with severe stroke and help them recover.
