肺癌CT征象和PET/CT最大标准化摄取值与Ki-67表达的关系

摘　　要：目的探讨肺癌CT征象与氟代脱氧葡萄糖(18F-FDG)正电子发射计算机断层显像(PET)/CT的最大标准化摄取值(SUVmax)和核抗原Ki-67阳性表达情况之间的关系。方法回顾性分析本院初诊肺癌患者96例,其中腺癌57例,鳞癌14例,神经内分泌肿瘤25例,均采用免疫组织化学法获得核抗原Ki-67表达情况,并根据其百分比分为Ki-67高表达组和低表达组。比较2组间CT征象、SUVmax的差异。结果淋巴结转移及多发淋巴结转移两个征象在Ki-67低表达组和高表达组之间差异有统计学意义(P<0.05);对比2组患者一般特征、病变位置、长径、密度、边界、形状、分叶征、毛刺征、钙化征、血管集束征、支气管充气征、空泡征、空洞征、支气管截断征、胸膜凹陷征、胸膜增厚征、胸腔积液,上述征象差异无统计学意义(P>0.05)。Ki-67高表达组肺癌的SUVmax摄取值更高。结论SUVmax值可在一定程度上反映肺癌Ki-67的表达情况,肺癌Ki-67高表达患者的SUVmax高于Ki-67低表达患者,肺癌Ki-67高表达患者更容易出现淋巴结转移及多发淋巴结转移。Objective To investigate the relationship between CT signs of lung cancer,maximum standardized uptake value(SUVmax)of 18F-FDG PET/CT and positive expression of nuclear antigen Ki-67.Methods Retrospectively analyze 96 patients with newly diagnosed lung cancer in our hospital,including 57 cases with adenocarcinoma,14 cases with squamous cell carcinoma,and 25 cases with neuroendocrine tumors.The expression of nuclear antigen Ki-67 were detected by immunohistochemical method,and according to the percentage,the patients were divided into Ki-67 high expression group and low expression group.The differences in CT signs and SUVmax between the two groups were compared.Results The two signs of lymph node metastasis and multiple lymph node metastasis were significantly different between the Ki-67 low expression group and high expression group.As for the general characteristics,lesion location,length,density,boundary,shape,and lobulation signs,spiculation sign,calcification sign,vascular cluster sign,air bronchus sign,vacuolization sign,cavity sign,bronchectomy sign,pleural depression sign,pleural thickening sign,and pleural effusion,there were no significant difference.The SUVmax uptake value of lung cancer in the Ki-67 high expression group was higher.Conclusion The SUVmax value can reflect the expression of Ki-67 in lung cancer to a certain extent.The SUVmax of patients with high expression of Ki-67 in lung cancer is higher than that of patients with low expression of Ki-67,and patients with high expression of Ki-67 in lung cancer are more prone to lymph node metastasis and multiple lymph node metastasis.
