益气利湿解毒方联合常规西药治疗脾虚伴湿热型宫颈高危HPV感染的临床观察

摘　　要：目的研究中药联合常规西药治疗脾虚伴湿热型宫颈高危HPV(HR-HPV)感染的临床疗效。方法选取63例符合中医带下病脾虚伴湿热型宫颈HR-HPV阳性患者,按数字表法随机分为两组,治疗组31例,对照组32例。两组均采取常规西药干扰素阴道用药;治疗组基于对照组的治疗基础上,配合口服摄入益气利湿解毒方。比较两组治疗后高危型HPV转阴、中医证候以及微生态指标。结果治疗组治疗后转阴18例,有效10例,转阴率58.1%,总有效率90.32%;对照组进行治疗后,转阴10例,有效11例,转阴率31.3%,总有效率为62.50%,两组差异有统计学意义(P<0.05);两组中医症状都有一定的改善,治疗组的优化更加显著(P<0.05);治疗组经过治疗后,菌群密度向正常状态的Ⅱ~Ⅲ级集中(P<0.05),阴道pH值向正常状态的3.8~4.5集中,Nugent指标有一定的降低(P<0.05),而对照组菌群密度和pH值变化、Nugent指标变化不明显(P>0.05),说明治疗组经治疗后,菌群状态和阴道酸碱度、Nugent指标好转。结论干扰素阴道用药联合益气利湿解毒方对高危HPV感染有良好效果,有益于改善临床症状和阴道微生态。Objective To study the clinical efficacy of traditional Chinese medicine combined with conventional western medicine in the treatment of spleen deficiency with damp heat cervical high-risk HPV(HR-HPV).Methods 63 cases of cervical HR-HPV positive patients with spleen deficiency and damp heat in accordance with traditional Chinese medicine were randomly divided into two groups,31 cases in the observation group and 32 cases in the control group.For both groups,conventional western medicine interferon was used vaginally;Based on the treatment of the control group,the observation group was combined with oral intake of Yiqi Lishi Jiedu Recipe.The negative conversion of high-risk HPV,TCM syndrome and microecological indexes were compared between the two groups.Results In the treatment group,18 cases turned negative and 10 cases were effective.The negative conversion rate was 58.1%and the total effective rate was 90.32%;After treatment in the control group,10 cases turned negative and 11 cases were effective.The negative conversion rate was 31.3%.The total effective rate was 62.50%.There was significant difference between the two groups(P<0.05);The TCM symptoms of the two groups were improved to some extent,and the optimization of the treatment group was more significant(P<0.05);After treatment,the flora density in the treatment group concentrated to the levelⅡ~Ⅲof the normal state(P<0.05),the vaginal pH value concentrated to the level 3.8~4.5 of the normal state,and the Nugent index decreased to a certain extent(P<0.05),while the flora density,pH value and Nugent index in the control group did not change significantly(P>0.05),indicating that the flora status,vaginal pH and Nugent index in the treatment group improved after treatment.Conclusion The combination therapy of traditional Chinese and Western medicine studied in this paper has a good effect on high-risk HPV infection,is beneficial to improve clinical symptoms and vaginal microecology,and has important popularization value.
