摘　　要：目的探讨全数字化乳腺摄影(FFDM)对男性乳腺癌的临床诊断价值。方法选取术前行FFDM检查并根据乳腺影像报告和数据系统(BI-RADS)分类标准诊断为乳腺癌,术后经病理证实的14例男性乳腺癌患者的影像及临床资料,分析本病的X线表现、临床特点及诊断要点。结果14例病灶均为浸润性导管癌。病变均位于乳晕后方,其中12例位于乳晕正后方,2例位于乳晕后略偏外上。4例病灶边缘模糊、浸润生长、呈不规则形,其中2例伴有泥沙样钙化;其余10例病灶边界清晰,6例不规则形,边缘有毛刺,其中1例伴有细小颗粒样钙化,4例呈卵圆形。所有病变均呈略高及高密度,直径3.1~8.6 cm,平均5.8 cm。14例均伴有乳头凹陷及周围皮肤增厚;6例伴有腋窝淋巴结转移;4例伴有乳腺发育。结论男性乳腺癌发病率低,在X线上主要表现为乳晕后软组织肿块,常伴发乳头凹陷、皮肤增厚及腋窝淋巴结肿大,FFDM对该病诊断正确率高。Objective To explore the clinical diagnostic value of full-field digital mammography(FFDM)for male breast cancer.Methods The X-ray findings,clinical features and diagnostic key points were retrospectively analyzed in 14 male breast cancer,which were diagnosed according to the breast imaging reporting and data system(BI-RADS)classification criteria and confirmed by surgical pathology.Results All 14 cases were diagnosed as invasive ductal carcinoma.All the lesions were located behind areola,including 12 cases in just behind areola and 2 cases slightly in the upper outer quadrant.The shape of 4 lesions was fuzzy,infiltrating and irregular,of which 2 cases were accompanied with sediment-like calcification.The other 10 cases had a clear boundary,of which 6 cases had irregular shape with spiculation,1 case was accompanied with fine granular calcification,and the other 4 cases were oval.The lesions were slightly higher or high density.The diameters of the lesions ranged from 3.1 cm to 8.6 cm with an average of 5.8 cm.All 14 cases had skin thickening with nipple retraction.6 cases were accompanied with axillary lymph node metastasis,and 4 cases associated with breast development.Conclusion The incidence rate of male breast cancer is low.On X-ray,the main manifestation is soft tissue mass behind areola,often accompanied with nipple retraction,skin thickening and axillary lymph node enlargement.FFDM has a high diagnostic accuracy for the disease.
