摘 要:视神经脊髓炎谱系疾病(NMOSD)是一种中枢神经系统疾病,以脱髓鞘和轴突损伤为特征,主要累及视神经和脊髓,患者常表现为与视神经炎或脊髓炎相关的症状和典型的复发病程。有些患者表现为较不常见的脑干症状,如恶心和呕吐,然而呕吐患者常首选消化内科就诊,首诊医师易关注呕吐症状,而忽视其病因,导致无典型症状的NMOSD患者漏诊。本文报道1例以反复恶心呕吐为特征的视神经脊髓炎谱系疾病,总结出当呕吐患者反复治疗效果欠佳时,需考虑其他学科疾病可能,使非典型NMOSD患者得到尽早诊断及治疗,避免漏诊及误诊的发生。Neuromyelitis optica spectrum disorders(NMOSD)is a central nervous system disease characterized by demyelination and axonal injury,mainly involving the optic nerve and spinal cord.Patients often show symptoms associated with optic neuritis or myelitis and a typical course of recurrence.Some patients often show less common brainstem symptoms,such as nausea and vomiting,but vomiting patients often prefer to see a doctor in the Department of Gastroenterology.The first physician is easy to focus on vomiting symptoms,while ignoring the cause,leading to missed diagnosis of NMOSD patients with atypical symptoms.This paper reports a case of NMOSD characterized by repeated nausea and vomiting.It is concluded that when the repeated treatment effect of vomiting patients is poor,it is necessary to consider the possibility of diseases in other disciplines,so that more atypical NMOSD patients can be diagnosed and treated early,in order to avoid missed diagnosis and misdiagnosis of the disease and prevent the potential poor prognosis.
