全子宫切除伴或不伴双侧附件切除术治疗子宫肉瘤疗效及预后比较分析

摘　　要：目的分析并比较全子宫切除伴或不伴双侧附件切除术治疗子宫肉瘤疗效及预后。方法选取80例子宫肉瘤患者,将40例伴双侧附件切除患者作为对照组,将不伴双侧附件切除患者40例作为试验组。比较两组患者治疗效果。结果试验组患者手术时间(124.54±10.25)min长于对照组的(113.25±13.24)min,差异有统计学意义(P<0.05)。试验组术中出血量、住院时间、拔除导尿管时间及肠胃功能恢复时间与对照组比较,差异无统计学意义(P>0.05)。手术前,两组阻力指数(RI)、脉动指数(PI)、收缩期最大峰值流速(Vmax)及舒张末最低流速(Vmin)指标比较,差异无统计学意义(P>0.05);手术后,两组RI、PI、Vmin及Vmax指标比较,差异有统计学意义(P<0.05)。结论子宫肉瘤患者伴双侧附件切除术治疗时间较长,但可改善患者术后卵巢动脉血流参数,具有推广价值。Objective To compare and analyze the efficacy and prognosis of total hysterectomy with or without bilateral adnexectomy on uterine sarcoma.Methods 80 patients with uterine sarcoma were selected,40 patients with bilateral adappendix resection were selected as control group,and 40 patients without bilateral adappendix resection were selected as experimental group.The therapeutic effect of the two groups was compared.Results The operation time of experimental group(124.54±10.25)min was longer than that of control group(113.25±13.24)min,and the difference was statistically signifi cant(P<0.05).There was no significant difference between the experimental group and the control group in intraoperative blood loss,hospital stay,catheter removal time and gastrointestinal function recovery time(P>0.05).Before operation,there were no significant differences in resistance index(RI),pulsation index(PI),systolic peak flow velocity(Vmax)and end-diastolic minimum fl ow velocity(Vmin)between the two groups(P>0.05).After operation,there were statistically significant differences in RI,PI,Vmin and Vmax between the two groups(P<0.05).Conclusion The treatment time of patients with uterine sarcoma accompanied by bilateral adannexectomy is relatively long,but the parameters of ovarian artery blood flow in patients after surgery are improved,which has the value of popularization.
