恒牙根尖周炎伴根尖开放治疗的疗效分析

摘　　要：目的:对比iRoot BP Plus根尖屏障术和Vitapex根尖诱导成形术治疗成人恒牙根尖周炎伴根尖开放的临床疗效。方法:纳入符合条件的40颗根尖周炎伴根尖开放的成人恒牙,分为iRoot BP Plus根尖屏障术组(n=21)及Vitapex根尖诱导术组(n=19),比较术后3、6、12、24个月两组的疗效、治疗周期、根尖孔直径及牙根长度的变化。结果:治疗3、6个月后,两组疗效均有统计学差异(P<0.05),iRoot BP Plus组疗效优于Vitapex组;但12、24个月后两组疗效均无统计学差异。iRoot BP Plus组治疗周期较Vitapex组短(P<0.05)。iRoot BP Plus组治疗前后的根尖孔直径及牙根长度均无统计学差异;Vitapex组治疗前后的根尖孔直径有统计学差异(P<0.05),牙根长度无统计学差异。结论:iRoot BP Plus根尖屏障术与Vitappex根尖诱导成形术均有利于治疗成人恒牙根尖周炎伴根尖开放,iRoot BP Plus组治疗周期短,根尖周组织愈合快,而Vitapex有利于促进根尖孔闭合但不能显著增加牙根长度。Objective:To compare the clinical efficacy of iRoot BP Plus apical barrier and Vitapex apexification in the treatment of adult permanent teeth with periapical periodontitis and apical opening teeth.Methods:This study included 40 adult permanent with apical periodontitis and apical opening.The teeth were divided into iRoot BP Plus apical barrier group(n=21)and Vitapex apexification group(n=19).The efficacy of 3,6,12,and 24 months after treatment, treatment period, changes in apical foramen diameter and root length were compared.Results:There was statistical difference in the efficacy of treatment between two groups at 3 months and 6 months after treatment respectively, the efficacy of the iRoot BP Plus group was better than that of the Vitapex group(P<0.05),but there was no statistical difference between two groups 12 months and 24 months after treatment.The treatment period of the iRoot BP Plus group was shorter than Vitapex group(P<0.05).There was no significant difference in the apical foramen diameter and root length before and after treatment in the iRoot BP Plus group.There was significant difference in the apical foramen diameter before and after treatment in the Vitapex group(P<0.05),but no significant difference in root length.Conclusion:Both iRoot BP Plus apical barrier and Vitapex apexification are effective treatments for adult permanent teeth with periapical periodontitis and apical opening.Compared with Vitapepx, iRoot BP Plus has shorter treatment period and faster periapical tissue healing.Vitapex could promote apical foramen closure but could not significantly increase root length.
