摘 要:目的 探讨经腹超声引导下胰腺占位穿刺活检对胰腺癌的诊断价值。方法 以术后病理诊断为金标准,分析不同病灶位置、不同病灶直径对取材满意度的影响,以及经腹超声穿刺活检诊断的准确率、敏感性和特异性、活检后的并发症发生情况。结果 不同病灶位置和不同病灶直径患者取材满意度比较,均无显著性差异(P<0.05)。经腹超声活检与术后病理结果诊断的一致性较好(Kappa=0.694,P<0.001)。经腹超声活检诊断准确率为96.08%(98/102),敏感性为96.88%(93/96),特异性为83.33%(5/6)。经腹超声活检后并发症发生率为7.84%,未发生穿孔、感染、胰瘘、肿瘤种植等严重的并发症。结论 经腹超声引导下胰腺占位穿刺活检与术后病理诊断的一致性较好。诊断的准确率、敏感性、特异性均较高,对胰腺癌具有较高的诊断价值,且并发症发生率较少,安全性较高,活检成功率受病灶位置和病灶直径影响较小,值得临床上推广使用。Objective To explore the diagnostic value of transabdominal ultrasound-guided percutaneous biopsy of pancreatic space occupying lesions in pancreatic cancer. Methods Postoperative pathological diagnosis was as the gold standard, the effects of different location and diameter of lesions on the satisfaction of biopsy were analyzed, as well as the accuracy, sensitivity and specificity of transabdominal ultrasound biopsy diagnosis, the occurrence of complications after biopsy were analyzed. Results There was no significant difference in the satisfaction of patients with different lesion locations and diameters(P<0.05). Transabdominal ultrasound biopsy was consistent with postoperative pathological diagnosis(Kappa=0.694, P<0.001). The diagnostic accuracy of transabdominal ultrasound biopsy was 96.08%(98/102), the sensitivity was 96.88%(93/96), the specificity was 83.33%(5/6). The incidence of complications was 7.84%. There were no serious complications such as perforation, infection, pancreatic fistula, tumor implantation. Conclusion Transabdominal ultrasound-guided percutaneous biopsy of pancreatic masses is consistent with postoperative pathological diagnosis. The accuracy, sensitivity, specificity and positive predictive value of diagnosis are high. It has high diagnostic value for pancreatic cancer, with less complications and high safety. The material is less affected by the location and diameter of the lesion. It is worth popularizing in clinical practice.
