摘  要:目的分析SRK/T公式、Barrett UniversalⅡ公式及正视验证光学(Emmetropia verifying optical,EVO)公式在长眼轴白内障患者人工晶体(IOL)度数计算中的准确性及稳定性。方法收集自2019年7月至2022年1月于新疆医科大学第一附属医院行白内障超声乳化摘除联合人工晶体(Phaco+IOL)植入术且眼轴长度(AL)≥26 mm的病人共95例135眼。根据AL将患者分为3组,A组(26 mm≤AL<28 mm)39例53眼,B组(28 mm≤AL<30 mm)41例53眼,C组(AL≥30 mm)21例29眼。分别计算SRK/T公式、Barrett UniversalⅡ公式及EVO公式的平均屈光误差(mean error,ME)及平均绝对屈光误差(mean absolute error,MAE)并进行统计学分析。结果SRK/T公式、Barrett UniversalⅡ公式及EVO公式产生的屈光误差普遍以正值为主,表明长眼轴患者术后普遍出现远视漂移。3种公式的MAE在B组和C组中有统计学差异(P<0.01)。在进行两两比较及计算各公式在0.5 D及1.0 D屈光阈值范围内百分比后显示,Barrett UniversalⅡ公式及EVO公式的准确性优于SRK/T公式。Spearman秩相关分析显示EVO公式的预测稳定性较好,受AL影响不明显(P>0.05)。结论长眼轴白内障患者术后普遍出现远视漂移,且随AL延长,屈光误差进一步增加。EVO公式的预测准确性和稳定性良好,故在长眼轴白内障患者IOL计算中更推荐使用EVO公式。ObjectiveTo analyze the accuracy of the SRK/T formula,Barrett UniversalⅡformula,and Emmetropia Verifying Optical(EVO)formula in the calculation of intraocular lens(IOL)power in cataract patients with long axial length.MethodsA total of 135 eyes of 95 patients with axial length(AL)≥26 mm underwent Phaco+IOL implantation in the hospitalfrom July 2019 to January 2022 were enrolled.According to AL,the patients were divided into 3 groups:group A(26 mm≤AL<28 mm)39 cases with 53 eyes,group B(28 mm≤AL<30 mm)41 cases with 53 eyes,group C(AL≥30 mm)21 cases 29 eyes.The mean error(ME)and mean absolute error(MAE)of the SRK/T formula,Barrett UniversalⅡformula,and EVO formula were calculated and statistically analysis.ResultsThe refractive errors generated by SRK/T formula,Barrett UniversalⅡformula and EVO formula were generally positive,indicating that hyperopic drift were common in patients with long axial length after surgery.The MAE of the three formulas were significantly different between group B and group C(P<0.01).The accuracy of Barrett UniversalⅡformula and EVO formula was better than SRK/T formula after the pairwise comparison and calculation of the percentage of each formula within the refractive threshold range of 0.5 D and 1.0 D.Spearman rank correlation analysis showed that the prediction stability of EVO formula was good,and it was not significantly affected by AL(P>0.05).ConclusionHyperopic drift is common after cataract surgery in patients with long-axial length,and the refractive error increases further with the extension of AL.The accuracy and stability of the EVO formula is good,so the EVO formula is more recommended in the calculation of IOL in cataract patients with long axial length.
